Dear Ms. Jewell:
Enclosed please find for filing an original and seven (7) copies of this leffer, which provides
supplemental information regarding i-wireless, LLC's ("i-wireless") pending Application for Limited
Designation as an Eligible Telecommunications Carrier ("ETC") in the State of Idaho.
i-wireless hereby requests that its designation as an ETC include the authority to participate in and
receive reimbursement from the,Idaho Telephone Service Assistance Program (ITSAP). Pursuant to this
authority, i-wireless will comply with applicable ITSAP regulations, including but not limited to required
monthly reporting as well as execution of a Memorandum of Understanding with the Department of Health
and Welfare. i-wireless further commits to remit required ITSAP funds to the ITSAP Administrator.
i-wireless affirms its commitment to comply with all applicable Idaho requirements pertaining to
designation as an ETC. i-wireless will also comply with tribal notification requirements, including providing
notice of i-wireless' ETC designation to afflected tribes.
I have also enclosed an extra copy of this letter to be date stamped and retumed to me in the
enclosed, self-addressed, postage prepaid envelope. If you have any questions or if I may provide you with
