In accordance with Order No. 35794, dated May 30, 2023, Ziply Fiber Northwest, LLC
submits Attachments A and B. Attachment A consists of a series of photographs chronicling Ziply
Fiber’s burial of cable between pedestals, along with the Ziply Fiber internal trouble ticket
reporting completion of the job on June 9, 2023. Attachment B is a witness statement from a
neighbor attesting that cable undergrounding work was completed on the same date.
WHEREFORE, Ziply Fiber asks the Commission to close the formal complaint in this
docket.
