BEFORE THE IDAHO PUBLIC UTILITIES COMMISSION

INCORPORATED HAS CHANGED ITS NAME
TO FRONTIER COMMUNICATIONS NORTHWEST INc.

On December 8, 2010, Frontier Communications Northwest Inc. ("Frontier ) filed an
Application to amend Verizon Northwest Incorporated's Certificate of Public Convenience and
Necessity. Frontier Communications acquired the Certificate when Frontier obtained Verizon
Idaho operations and service area. In this filing, Frontier requested that the Commission amend
the Certificate to reflect that Verizon Northwest Incorporated has changed its name to Frontier
Communications Northwest Inc.
The Companies state that on July 8 , 2010, the Idaho Secretary of State issued an
amended Certificate of Authority, File Number C 139394, reflecting this name change. Having
formally changed its name with the Secretary of State, Frontier now requests that the
Commission recognize the corporate name change.
The Commission Staff recommended that the Commission approve this Application
without further procedure. Rather than republishing all the tariffs/price list catalogs with new
pages reflecting the name change, Frontier changed the title pages to reflect the name change.
Based on our review of the Application, we find that it is reasonable to issue an
amended Certificate of Public Convenience and Necessity to reflect the change in name from
Verizon Northwest Incorporated to Frontier Communications Northwest Inc.
ORDER
IT IS HEREBY ORDERED that Frontier s Application to amend the corporate holder
of Certificate of Public Convenience and Necessity No. 383, changing the named holder from
Verizon Northwest Incorporated to Frontier Communications Northwest Inc., is granted.
Frontier will be issued an amended Certificate reflecting this name change.

reconsideration within twenty-one (2l) days of the service date of this Order. Within seven (7)
days after any person has petitioned for reconsideration, any other person may cross-petition for
reconsideration. See Idaho Code ~ 61-626.
