On December 8, 2010, Frontier Communications Northwest Inc. filed an Application to
amend Verizon Northwest's Certificate of Public Convenience and Necessity. More specifically,
Frontier requested that the Commission amend this Certificate to reflect that Verizon Northwest
Incorporated has changed its name to Frontier Communications Northwest Inc.
IT IS HEREBY CERTIFIED that this Second Amended Certificate of Public Convenience
and Necessity is issued to Frontier Communications Northwest Inc. to provide local exchange and
interexchange services within parts of the State of Idaho. The Commission approved this name change
by Order No. 32142 issued in Case No. VZN-OO-Ol.
