On August 30, 2000, Verizon Northwest, Inc. ("Verizon ) filed an Application to
amend GTE Northwest's Certificate of Public Convenience and Necessity. More specifically,
V erizon requested that the Commission amend this Certificate to reflect that GTE Northwest
Incorporated has changed its name to Verizon Northwest, Inc.
IT IS HEREBY CERTIFIED that this Certificate of Public Convenience and
Necessity is issued to Verizon Northwest, Inc. to provide Title 61 regulated telecommunications
services, including basic exchange services, within parts of the state of Idaho. The Commission
approved this name change by Order No. 28525 issued October 3 , 2000, in Case No. VZN- T -00-
