QWEST CORPORATION'S NOTICE OF ERRATA TO UPDATED STATEMENT OF GENERALLY AVAILABLE TERMS AND CONDITIONS
Qwest Corporation, formerly known as U S WEST Communications, Inc. (“Qwest”), submits this Notice of Errata to its previously filed updated Statement of Generally Available Terms and Conditions (“SGAT”), along with the corrected SGAT itself, which is attached. Qwest respectfully requests that the Idaho Public Utilities Commission (“Commission”) allow this version of the SGAT to go into effect.
It recently came to Qwest’s attention that four sections were missing from the SGAT that was filed with the Commission on May 24, 2002. These sections were: Section 13.0 (Access to Telephone Numbers); Section 14.0 (Local Dialing Parity); Section 15.0 (Qwest Dex); and Section 16.0 (Referral Announcement). As the result of an apparent clerical error, Qwest inadvertently omitted these four sections from the May 24th SGAT.
The present filing corrects this error; the previously omitted sections can be found at pages 303-306 of the attached SGAT. The reinstated Sections 13, 14, 15 and 16 contain no changes from the SGAT that preceeded the May 24th SGAT. Qwest notes that because these sections were not properly deleted from the last filing (i.e., since the deletion of these sections did not appear as red-lined), and because these sections were never intended to be deleted, the attached SGAT does not show these sections as new language.
Qwest apologizes for any confusion its error may have caused, and respectfully asks that the Commission allow the newly corrected May 24th SGAT to go into effect.
