TARIFF FORMAT
Sheet Numbering - Sheet numbers appear in the upper right comer of the page. Sheets
are numbered sequentially. When a new sheet is added between sheets already in effect
a decimal is added. For example, a new sheet added between sheets l4 and l5 would be
l4.
Sheet Revision Numbers - Revision numbers also appear in the upper right comer of
each page. These numbers are used to determine the most current sheet version on file
with the APSC. For example, the 4th revised Sheet l4 cancels the 3rd revised Sheet l4.
Consult the Check Sheet for the sheet currently in effect.
Paragraph Numbering Sequence - There are seven levels of paragraph coding. Each
level of coding is subservient to its next higher level:

SECTION 1 - TECHNICAL TERMS AND ABBREVIATIONS
Access Line - An arrangement which connects the Customer s location to a United Services
Telephone switching center or point of presence.
Authorized User - A person, firm, corporation, or any other entity authorized by the Customer
to communicate utilizing the Carrier s service.
Available Usage Balance - The amount of usage remaining in a Debit Account at any particular
point in time. Each Debit Account begins with an initial usage amount which is depleted as
services provided by the Company are utilized by the Customer.
Called Party - The person, individual, corporation, or other entity whose telephone number is
called by the End User. For calls placed on an Collect Billing basis, the Called Party accepts
responsibility for payment of the charges for use of services provided by UST.
Commission - The Idaho Public Utilities Commission.
Company or Carrier - United Services Telephone, LLC ("UST") unless otherwise clearly
indicated by the context.
Customer - The person, firm, corporation or other entity which orders, cancels , amends or uses
service and is responsible for payment of charges and compliance with the Company s tariff.
End User - Any person, firm, corporation, partnership or other entity which uses the services of
the Carrier under the provisions and regulations of this tariff. The End User is responsible for
payment unless the charges for the services utilized are accepted and paid by another Customer.
Identification Number - A unique numerical code associated with each Phone Card Call

Incomplete call - A call in which no Called Station was reached by the Caller (i.e. busy signal
or no answer).
IPUC - Idaho Public Utilities Commission
LATA - Local access and transport area. A geographic area established by the US District Court
for the District of Columbia in Civil Action No. l7-, within which a local exchange company
provides communications services.
LEC - Local Exchange Company
Marketer - Any person, firm, corporation or other legal entity authorized by UST to market
Phone Cards to the Public.
Phone Card - A telephone calling card bearing a unique Identification Number and issued by
UST or a Marketer to a Subscriber to Phone Card Service in order to permit callers to access the
UST Telecommunications network. Phone Cards are sold in various time and dollar
denominations.
Phone Card Call- A single call to an Toll Free (i.e. 800/888) access number whereby a Caller
after entering an Identification Number, may utilize the Phone Card Service. Each time the Caller
makes a Phone Card Call, the Caller will be told the number of minutes or the dollar amount
remaining with respect to the entered Identification Number. The charges for a Phone Card Call
are billed to or debited from the Phone Card account.
Phone Card Service - A communications service provided by UST which permits a Caller
use a Phone Card in order to place interstate calls from any touch-tone phone and to access
various information services on a presubscription basis.
Subscriber - A party who (i) purchases a Phone Card; (ii) purchases additional calling time for
a Phone Card; or (iii) receives a Phone Card from UST or a Marketer in connection with a
promotional offering.

Time Deductions - Deductions in the number of minutes remaining on a Subscriber s Phone Card
caused by using Phone Card Service.
Personal Account Code - A pre-defined series of numbers to be dialed by the Customer or
Authorized User upon access to the Carrier s network which identifies the Debit Account from
which charges for service shall be debited and which validates the caller s authorization to use
the services provided.
Premises - A building or buildings on contiguous property.
Renewal - A method of replenishing a Phone Card account's Available Usage Balance with
additional minutes of use as authorized and paid for by the Customer.

Undertaking of United Services Telephone, LLC
1.1 United Services Telephone, LLC is a resale common carrier providing intrastate
communications long distance message toll telephone service to Customers for the
transmission and reception of voice, data, and other types of communications.
1.2 The Company offers intrastate telecommunications service in conjunction with
interstate service.
1.3 Long distance usage charges are based on the actual usage of United Services
Telephone, LLC network. Chargeable time begins when a connection is
established between the calling station and the called station. Chargeable time
ends when either party "hangs up" thereby releasing the network connection.
1.4 No charges apply for incomplete calls.
1.5 Service is provided twenty-four (24) hours per day, seven (7) days per week.

Limitations
Service is offered subject to the availability of the necessary facilities and
equipment, or both facilities and equipment, and subject to the provisions of this
tariff.
United Services Telephone, LLC reserves the right to discontinue or limit service
when necessitated by conditions beyond its control, or when the Customer is using
service in violation of provisions of this tariff, or in violation of the law.
The company does not undertake to transmit messages, but offers the use of
facilities when available, and will not be liable for errors in transmission or for
failure to establish connections.

Payment and Credit Regulations
Payment Arrangements
The Customer is responsible for payment of all charges for services and equipment
furnished to the Customer for transmission of calls via the Company. The
Customer agrees to pay to the Company any cost(s) incurred as a result of any
delegation of authority resulting in the use of his or her communications
equipment and/or network services which result in the placement of calls via the
Company. The Customer agrees to pay the Company or its authorized agent any
and all cost(s) incurred as a result of the use of the service arrangement, including
calls which the Customer did not individually authorize.
All charges due by the Customer are payable to the Company or any agency duly
authorized to receive such payments. Terms of payment shall be according to the
rules and regulations of the agency and subj ect to the rules of regulatory agencies
such as the IPUC. Any objections to billed charges must be promptly reported to
the Company or its billing agent. Adjustments to Customers I bills shall be made
to the extent that circumstances exist which reasonably indicate that such changes
are appropriate.
Charges for installations , service connections, moves, and rearrangements, where
applicable, are payable upon demand by the Company or its authorized agent. The
billing thereafter will include recurring charges and actual usage as defined in this
tariff.

Payment Arrangements, cont'
The Customer shall be responsible for all calls placed by or through Customer
equipment by any person. In particular and without limitation to the foregoing,
the Customer is responsible for any calls placed by or through the Customer
equipment via any remote access features. The Customer is responsible for all
calls placed via their authorization code as a result of the Customer s intentional
or negligent disclosure of the authorization code.
Deposits
The Company does not require deposits.
Advance Payments
The Company reserves the right to require an advance payment from a Customer.
Fixed recurring charges for certain service may be billed in advance, while usage
charges are billed in arrears.

Taxes
The Company shall charge the Customer an amount sufficient to recover any
governmental assessments, fees, license, or other similar taxes or fees imposed
upon the Company.
For Debit Service, taxes or fees shall be included in the rates and charges
stated in the Company s rate schedule for this service.
For all other services offered by the Company, taxes and fees shall be
added pro-rate, insofar as practical, to the rates and charges stated in the
Company s rate schedules and listed as separate line items on the
Customer s bill for services provided.
Late Payment Charge and Cost of Collection
A late fee of l.5% per month will be charged on outstanding balance which is
past due by thirty days. In the event that the Company incurs fees or expenses
including attorney s fees, collecting or attempting to collect, any charges owed to
the Company, the Company may charge the Customer all such fees and expenses
reasonably incurred after assigned to an outside collection agency.
Return Check Charge
return check charge of $20.00 will be assessed for checks returned for
insufficient funds. Any applicable return check charges will be assessed according
to the terms and conditions of the billing entity (i.e. local exchange company
and/or commercial credit card company) and pursuant to Idaho law and
Commission regulations.

2.4 Use
Services provided under this tariff may be used for any lawful purpose for which the
service is technically suited.
Terminal Equipment
The Company facilities and service may be used with or terminated in
Customer-provided terminal equipment or Customer-provided communications systems
such as a telephone set, PBX or key system. Such terminal equipment shall be furnished
and maintained at the expense of the Customer, except as otherwise provided. The
Customer is responsible for all costs at his or her premises, including personnel, wiring,
electrical power, and the like, incurred in the use of the Company s service. When such
terminal equipment is used, the equipment shall comply with the generally accepted
minimum protective criteria standards of the telecommunications industry as endorsed by
the Federal Communications Commission.

Application for Service
The Company reserves the right to require Customers to make application(s) for service
in writing using forms provided by the Company. Upon acceptance of an application for
service by the Company, all applicable provisions in the Company s tariffs, as amended
from time-to-time which are lawfully on file, become the agreement for service between
the Company and the Customer. Requests for additional service and changes to service
upon acceptance by the Company become a part of the agreement for service and changes
to the service, provided that each item of additional service shall be subject to the
applicable minimum term of service. Acceptance or use of service offered by the
Company shall be deemed an application for such service and an agreement by the
Customer to subscribe to, use, and pay for such service and in accordance with the
applicable tariffs of the Company, as amended from time to time, which are lawfully on
file. Any change in rates or other tariff provisions which are lawfully made shall be
deemed to modify all agreements for service affected by such changes without further
notice by Company to the Customer.
Cancellation by Customer
Customer may cancel service by providing 5 days written or verbal notice to the
Company.

Refusal or Discontinuance by Company
United Services Telephone, LLC may refuse or discontinue service under the following
conditions. Unless otherwise specified, the Customer will be given five (5) days written
notice and allowed a reasonable time to comply with any rule or remedy any deficiency.
Without notice for non-compliance with and/or violation of any State or municipal
law, ordinance or regulation pertaining to telephone service.
For the use of telephone service for any other property or purpose other than that
described in the application.
Without notice for non-compliance with and/or violation of the Commission
regulations or the Company s rules and regulations on file with the Commission.
8.4 For non-payment of bills for telephone service.
Without notice in the event of Customer use of service in such a manner as to
adversely affect the Company s service or the Company s service to others.
Without notice in the event of tampering with the service furnished and owned by
the Company.
Without notice in the event of unauthorized or fraudulent use of service.
Whenever service is discontinued for fraudulent use of service, the Company may,
before restoring service, require the Customer to make, at his own expense, all
changes in facilities or equipment necessary to eliminate illegal use and to pay an
amount reasonably estimated as the loss in revenues resulting from such fraudulent
use.

Refusal or Discontinuance by Company, Cont'
For failure of the Customer to make proper application for service.
For Customer s breach of the contract for service between the Company
and the Customer.
Without notice when necessary for the Company to comply with any order
or request of any governmental authority having jurisdiction.
Credit Allowance for Interruption of Service
Credit allowances for interruptions of service caused by service outages or deficiencies
are limited to the initial minimum period call charges for re-establishing the interrupted
call.
10 Interconnection
Service furnished by United Services Telephone, LLC may be connected with the services
or facilities of other carriers. Such service or facilities, if used, are provided under the
terms , rates and conditions of the other carrier. The Customer is responsible for all
charges billed by other carriers for use in connection with United Services Telephone
LLC's service , such as the basic local line used to dial the Company. Any special
interface equipment or facilities necessary to achieve compatibility between carriers is the
responsibility of the Customer.

Inspection, Testing and Adjustment
Upon reasonable notice, the facilities provided by the Company or its underlying carrier
shall be made available to the carrier for tests and adjustments as may be deemed
necessary by the carrier for maintenance. No interruption allowance will be granted for
the time during which such tests and adjustments are made when the interruption is less
than twenty-four consecutive hours.
Tests, Pilots, Promotional Campaigns and Contests
The Company may conduct special tests or pilot programs and promotions to demonstrate
the ease of use, quality of service and to promote the sale of its services. Such tests
programs and promotions will be filed with the IPUC prior to being implemented.
Reservation of Toll Free "800/888" Numbers
The Company will make every effort to reserve Toll Free "800/888" vanity numbers for
customers, but makes no guarantee or warranty that the requested number(s) will be
available.

Calculation of Distance
Usage charges for mileage sensitive services vary based on the type of service subscribed
to by the Customer. For services utilizing switched access, mileage measurements for rate
schedules are based on the distance in airline miles between rate centers associated with
the originating and terminating stations. For services utilizing dedicated access, mileage
measurements for rate schedules are based on the distance in airline miles between the
UST network access point associated with the station utilizing Dedicated Access
Origination/Termination and the rate center associated with the called/calling station.
The distance between the originating and terminating points is calculated by using the "
and "H" coordinates of the serving wire centers as defined by BellCore (Bell
Communications Research), in the following manner:

Formula:
Obtain the "V" and "H" coordinates for the serving wire center or network
access point serving the Customer s location and the called/calling station.

Call Timing
Long distance usage charges are based on the actual usage of the Company
network. Chargeable time begins when the calling and the called station are
connected.
Chargeable time ends when the calling service point terminates, thereby releasing
the network connection.
Unless otherwise specified in this tariff, usage is measured in one (l) minute
increments for billing purposes. Partial usage will be rounded up to the next
highest whole minute. All calls are rounded to the next highest billing increment.
Any partial cents per call will be rounded up to the next highest cent.
Unless flat rated, usage charges are determined by the time of day rate periods and
minutes of use within each rate period. The rate period is determined by the time
and day of call originating at the Customer s Location.
5 No charges apply to unanswered calls.
Volume Based Plan Enrollment
Placement into various Plans is dependent upon prior actual monthly usage or
estimated monthly usage. Movement from the initial plan of enrollment to a
higher usage, lower rated plan will be effectuated if requested by a Customer.
Customers will be notified via bill insert that various volume-based rate plans are
available upon request.

Customers who choose the Block of Time Calling Option receive steeply
discounted long distance rates in exchange for a commitment to a fixed minimum
monthly network access plan. Network access is provided by using a toll-free
access number provided by the Company. Service is billed monthly in arrears
on the Customer s local telephone company bill.

Phone Card Service
11.General
Phone Card Service is voice grade switched telecommunications service
that allows an end user to place calls charged to cards issued by the
Company. The end user accesses the network by dialing an Toll Free (i.e.
800/888) number printed on the back of the card via a touch-tone
telephone. Cards will be offered to Customers on a first come, first served
basis. The number of cards offered by the Company will be subject to
technical limitations.
11.Unit Value
Phone Cards may be obtained from the company or authorized agent in
various denominations with a per unit value which is inclusive of all taxes.
One unit equals one minute (or fraction thereof) of domestic calling. The
rates apply 24 hours per day, 7 days a week. The value of the
telecommunications service (in units or dollars) will be indicated on the
card. Credit allowances for failure of service will be granted in accordance
with the terms set forth in this tariff.
Phone Cards are offered in a variety of denominations with a per unit
value not to exceed $0., inclusive of all taxes. One unit equals one
domestic minute.

11.Exclusions
The following types of calls cannot be completed with Phone Card
Service:
(a)
(b)
(c)
Calls to 700 , 800, 888, 900 and 950 numbers;
Calls to Directory Assistance; and
Operator Assisted Calls. (Call completion assistance will be
provided by Customer service personnel at no charge to the
Customer if the Customer encounters difficulty in completing a
call).
11.4 Card DepletionlRenewal and Expiration
Phone Cards will be reduced and depleted proportionate to Customer
usage. At the beginning of each call, the user will be notified as to the
amount of minutes available on the card. Customers will be given a notice
one minute before the balance is depleted. When the available time is
depleted, the card will be terminated.
Phone Cards are non-refundable and will expire on the date specified on
the card or one (l) year from the date of first use, unless a different term
is specifically requested by the Customer.
11.Special Responsibility
The Company is not responsible for theft, loss or unauthorized use of any
Phone Card or the associated Personal Identification Number (PIN).
