SECTION 1.0 - DEFINITIONS
For the purpose of fu tariff, the following defiiùtions wil apply:
Access Line - An arrangement which connects the Customer's location to a carrier's switchig center or
point of presence.
Account Codes - Optional, Customer-defied digits that alow the Customer to identi the individual user,
department or client associated with a call. Account Codes appear on the Customer bil.
Advance Payment - Par or al of a payment required before the start of service.
Authonzed User - A person, fum, corporation, or any other entity authorized by the Customer to
communcate utiing the Company's service.
Business - A class of service provided to individuals engaged in business, firms, partnerships, corporations,
agencies, shops, works, tenats of office buidings, and individuals practicing a profession or operatig a
business who have no offices other than their residences and where the use of the service is primary or
substatialy of a business, professional or occupationa nature.
Commssion - Idaho Public Utities Commsion.
Company or Carier - Unite Private Networks, L.L.c., uness otherwise clearly indicated by the context
Customer - The person, fum, corporation or other entity which orders, cancels, amends or uses service and is
responsible for payment of charges and complice with the Company's taif.
Deposit - Refers to a cash or equivalent of cash secrity held as a guarantee for payment of the charges.

DID Trunk - A form of local switched access that provides the abilty for an outside party to call an internal
extension directly without the intervention of the Company operator.
Dial Pulse (or "DP") - The pulse ty employed by rota di station sets.
Dual Tone Multi-Frequency (or "DTMF") - The pule tye employed by tone dial station sets.
End User - Any person, fim, corporation, partnership or other entity which uses the services of the
Company under the provisions and regutions of fu taf. The End User is responsible for payment
unless the charges for the services utid are accepted and paid for by another Customer.
End Office - With respect to each NP A-NX code prefi assigned to the Company, the location of the
Company's "end offce" for purposes of fu tari shal be the point of interconnection associated with that
NP A-NXX code in the Local Exchange Routig Guide ("LERG"), issued by Bellcore.
Hearg Impaired - Those persons with communcation impairments, including those hearing impaired,
deaf, deaf/blid, and speeh impaired persons who have an impairment that prevents them from
communcatig over the telephone without the aid of a telecommuncations device for the deaf.
Hunting - Routes a call to an idle station lie in a prearranged group when the caled station lie is busy.
In-Only - A service attibute that restricts outward dia access and routes incomig cal to a designated
anwer point
!XC or Interexchange Carer - A long distace telecommuncations services provider.

LATA - A Local Access and Tranport Area establihed pursuant to the Modifcation of Final Judgement
entered by the United States District Court for the District of Columbia in Civil Action No. 82-0192; or any
other geographic area designated as a LATA in the National Exchange Carrier Association, Inc. Tariff F.c.c.

LEC - Local Exchange Company
Minimum Point of Presence (tlMPOptl) - The mai telephone closet in the Customer's building.
Monthly Recuring Charges - The monthy charges to the Customer for services, facilties and equipment,
which contiue for tlie agreed upon duration of the service.
Multi-Frequency or (tlMFtl) - An inter-machie pulse tye used for signalg betwee telephone switches, or
between telephone switches and PBX/key systems.
Non-Recuring Charge (tlNRCtl) - The intil charge, usually assessed on a one-tie basis, to intite and
establih servce.
Other Telephone Company - An Exchange Telephone Company, other than the Company.
PBX - Private Branch Exchange
Premises - A buidig or buidigs on contiguous property.
Recung Charges - The monthy charges to the Customer for servces, facilties and equipment which
contiue for the agreed upon duration of the service.
Residence or Residential- A class of service furnhed to a Customer at a place of dwellg where the actual
or obvious us is for domestic puroses.

Service Commencement Date - The first day following the date on which tlie Company notiies tlie
Customer tliat tlie requested service is available for use, uness extended by tlie Customer's refusal to accept
service which does not conform to stadards set forth in the Service Order and this taiff, in which case the
Service Commencement Date is the date of the Customer's acceptace. The Company and Customer may
mutually agree on a substitute Service Commencement Date.
Servce Order - The written request for services executed by the Customer and the Company in the format
devised by the Company. The signg of an Order by the Customer and acceptace by the Company
intites the respective obligations of the parties as set forth therein and pursuant to th tariff, but the
duration of the service is calculated from the Service Commencement Date.
Telecommuncations Company or Provider - Used thoughout th tarif to mean Unite Private Networks,
L.L.c. uness clearly indicate otherwise by the text.
TBD - To Be Determined.
Two Way - A service attibute that includes outward dial capabilties for outbound call and can also be
used to carry inbound cal to a central point for furtler processing.
Usage Based Charges - Charges for miutes or messages traversing over local exchange facilties.
User or End User - A Customer, Joint User, or any other person authorized by a Customer to use servce
provider under th tarif.

SECTION 2.0 - RULES AND REGULATIONS
2.1 Undertaking of the Company
2.1.1 Scope
The Company undertaes to furnih communications service pursuant to the terms of tl1i
tariff in connection with one-way andj or two-way inormation tranmision origiatig
from points with the State of Idaho, and termiatig with a local callg area as defined
herein.
The Company is responsible under this tarif only for the services and facilties provided
. hereunder, and it assumes no responsibilty for any service provided by any other entity
that purchases access to the Company network in order to origiate or termate its own
services, or to communcate with its own Customers.
2.1.2 Shortage of Equipment or Facilities
(A) The Company reserves the right to lit or to allocate the use of exitig facilties,
or of additional facilties offered by the Company, when necessary because of lack
of facities, or due to some other cause beyond the Company's control.
(B) The funihig of servce under th taiff is subject to the avaiabilty on a
contiuig basis of al the necessar facities and is lited to the capacity of the
Company's facilties as well as facilties the Company may obta from other
carriers to fuh servce from tie to tie as requied at the sole dicretion of the
Company.

2.1.3 Terms and Conditions
(A) Service is provided on the basis of a mium period of at least one month, 24hours per day. For the purpose of computig charges in th tarif, a month is
considered to have th (30) days.
(B) Customers may be required to enter into writt servce orders which shal conta
or reference a specifc desciption of the servce ordered, the rates to be charged, the
duration of the services, and the terms and conditions in th tarif. Customers wi
also be requied to execute any other docuents as may be reasonably requested
by the Company.
(q Except as otherwise stated in the ta, at the expiration of the intial term specifed
in each Service Order, or in any extension thereof, service shal contiue on a month
to montli basis at the then curent rate uness termited by either party upon
proper notice. Any termiation shal not relieve the Customer of its obligation to
pay any charges incurred under the service order and th tarif prior to
termiation. The rights and obligations which by their natue extend beyond the
termiation of the term of the service order shal surive such termition.
(D) Service may be termated upon wrttn notice to the Customer if:
(1) the Customer is using the servce in violation of th ta; or
(2) the Customer is using the service in violation of the law.
(E) Th taif shall be interpreted and governed by the laws of the State of Idaho
without regard for its choice of laws proviion.

(F) Any Other Telephone Company may not interfere with the right of any person or
entity to obta service diecty from the Company. No person or entity shal be
required to make any payment, incur any penalty, moneta or otherwe, or
purchase any services in order to have the right to obtain service diectly from the
Company.
(G) Reserved for future us.
(H) The Company hereby reserves its rights to establih servce packages specifc to a
particular Customer. These contracts mayor may not be associated with volume
andj or term dicounts.

2.1.4 Limitations on Liabilty
(A) The liabilty of the Company for damages arising out of the furnihing of its
Service, including but not lited to mitaes, omisions, interruption, delay, or
errors, or other defects, representations, or use of these services or damages
arising out of the failure to furnih the service, whether caused by acts or
omission, shal be liited to the extension of allowances for interruption as set
forth in 2.7. The extension of such allowances for interruption shall be the sole
remedy of the Customer and the sole liabilty of the Company. The Company
wi not be liable for any direct, indirect, incidental, special, consequential, lost
profits, exemplary or punitive damages to Customer as a result of any Company
service, equipment or facilties, or the acts or omissions or negligence of the
Company's employees or agents.
(B) The Company's liabilty for wil misconduct, if establihed as a result of
judicial or admitrative proceedings, is not liited by th tariff. The
Company's liabilty, if any, with regard to delayed intallation of Company
facilties or commencement of service, shall not exceed $1,000. With respect to
any other claim or suit, by a Customer or by any others, for damage associated
with the ordering (including the reservation of any specifc number for use with a
service), intallation (including delays thereof), provision, termination,
maintenance, repai, interruption of restoration of any service or facilties offered
under this tariff, and subject to the provisions of Section 2.7, the Company's
liabilty, if any, shall be lited as provided herein.
(C) The Company shall not be liable for any delay or failure of performance or
equipment due to causes beyond its control, includig but not lited to: acts of
God, fire, flood, explosion or other catastrophes; and law, order, regulation,
direction, action or request of the United States government or of any other
government, including state and local governments having or claimg
jurisdiction over the Company, or of any department, agency, commision,
bureau, corporation or other intrmentality of anyone or more of these federal,
state, or local governments, or of any miltary authority; preemption of existig
service in complince with national emergencies; inurrections; riots; wars;
unavailabilty of rights-of-way or materials, or strikes, lockouts, work stoppages,
or other labor difficulties.

(D) The Company shall not be liable for (a) any act or omission of any entity
furnishing the Company or the Company's Customers facilties or equipment
used for or with the services the Company offers; or (b) for the acts or omisions
of common carriers or warehousemen. Except in 31.41.01 Rule 501, 502, and 503
of the IDAP A.
(E) The Company shall not be liable for any damages or losses due to the fault ofnegligence of the Customer or due to the failure or malfnction of Customer-
provided equipment or facilties.
(F) The Customer shall indemn and hold the Company harmless from any and all
loss, claims, demands, suits, or other action, or any liabilty whatsoever, whether
suffered, made, intituted, or asserted by any other party or person(s), and for
any loss, damage, or destruction of any property whether owned by the
Customer or others, caused or claimed to have been caused diectly or indirectly
by the intallation, operation; failure to operate, maintenance, removal, condition,
location, or use of intallation provided by the Company. The Company reserves
the right to require each Customer to sign an agreement acknowledgig
acceptance of the provisions of this section as a condition precedent to such
intallations.
(G) The Company shal not be liable for any defacement of or damage to Customer
premises resultig from the fuhig of services or equipment on such premies
or the intallation or removal thereof, unless such defacement or damage is
caused by gross negligence or wil misconduct of the Company's agents or
employees. No agents or employees of other participatig carriers shal be
deemed to be agents or employees of the Company.
(H) Notwithstanding the Customer's obligations, the Company shall be indemned,
defended, and held harmless by the Customer or by others authorizd by it to
use the service againt any clai, loss of damage arising from Customer's use of
services furnhed under this tariff, including: claim for libel, slander, invasion
of privacy or inringement of copyright arising from the material, data,
inormation, or other content tranmitted via the Company's service; and patent
inringement claim arising from combing or connectig the servce offered by
the Company with apparatus and systems of the Customer or others. All other
claims arising out of any act or omision of the Customer or others, in connection
with any service provided by the Company pursuant to th tarif.

(I) The entie liabilty of the Company for any claim, loss, damage or expense from
any cause whatsoever shall in no event exceed sums actually paid to the
Company by Customer for the specifc services giving rise to the claim, and no
such action or preceding againt the Company shall be commenced more than
one year after the service is rendered.
(J The Company makes no warranties or representations, express or implied,
including warranties of merchantabilty or fitness for a particular use, except
those expressly set forth herein.
(K) The Company shall not be liable for any act or omission of any other company or
compaiúes furnishing a portion of the service, or for damages associated with
service, channels, or equipment which it does not furnih, or for damages which
result from the operation of Customer-provided systems, equipment, facilties or
services which are interconnected with Company services. Except in 31.41.01
Rule 501, 502, and 503 of the IDAP A.
(L) The Company does not guarantee nor make any warranty with respect to service
intallations at locations at which there is present an atmosphere that is
explosive, prone to fie, dangerous or otherwise unuitable for such intallations.
The Customer shall indemn and hold the Company harmless from any and all
loss, claims, demands, suits or other action, or any liabilty whatsoever, whether
suffered, made, intituted or asserted by the Customer or by any other party, for
any personal injury to, or death of, any person or persons, or for any loss,
damage or destrction of any property, whether owned by the Customer or
others, caused or claimed to have been caused, directly or indirectly, by the
intalation, operation, failure to operate, maintenance, removal, presence,
condition, locations or use of service fuhed by the Company at such locations.

(M) Witl respect to Emergency Number 911 Service:
(a) Thi service is offered solely as an aid in handing assistance call in
connection with fie, police and other emergencies. The Company is not
responsible for any losses, clai, demands, suits or any liabilty whatsoever,
whether suffered, made, intituted or asserted by tle Customer or by any
other party or person for any personal injury to or death of any person or
persons, and for any loss, damage or destruction of any property, whetler
owned by the Customer or otlers, caused or claimed to have been caused by:
(1) mistakes, omissions, interruptions, delays, errors or other defects in tleprovision of this service, or (2) intallation, operation, failure to operate,
maintenance, removaL, presence, condition, location or use of any equipment
and facilties furnihing this service.
(b) The Company is not responsible for any iningement or invasion of the right
of privacy of any person or persons, caused or claimed to have been caused,
directly or indirectly, by the installation, operation, failure to operate,
maintenance, removal, presence, condition, occasion or use of emergency 911
service features and the equipment associated tlerewitl, or by any services
furnihed by the Company including, but not liited to, the identication of
tle telephone number, address or name associated with the telephone used
by the party or parties accessing emergency 911 service, and which arise out
of the negligence or other wrongf act of the Company, tle Customer, its
users, agencies or municipalities, or the employees or agents of anyone of
tlem.
(N) The Company's liabilty arising from errors or omissions in Directory Listigs,
otler tlan charged listig, shall be lited to the amount of actual impairment of
tle Customer's service and in no event shall exceed one-half the amount of tle
fied monthy charges applicable to exchange service affected during tle period
covered by the directory in which the error Qr omission occurs. In cases of
charged Directory Listigs, the liabilty of the Company shall be lited to an
amount not exceeding tle amount of charges for the charged litigs involved
during the period covered by the directory in which tle error or omission occurs.

(0) When a Customer witl a non-publihed telephone number, as defined herein,
places a call to the Emergency 911 Service, the Company wi release the name
and address of the callg party, where such inormation can be determied, to
the appropriate local governmental autl0rity responsible for the Emergency 911
Service, upon request of such governmental authority. By subscribing to service
under th taiff Customer acknowledges and agrees witl tle release of
inormation as described above.
(P) The included tariff language does not constitute a determiation by tle
Commsion that a liitation of liabilty imposed by the Company should be
upheld in a court of law. Acceptance for filg by the Commission recognes
tlat it is a court s responsibilty to adjudicate negligence and consequential
damage claims. It is also tle court s responsibilty to determine tle validity of
the exculpatory clause.
2.1.5 Notiication of Servce-Mfectig Activities
The Company wi provide tle Customer reasonable notication of servce-affectig
activities that may occur in norma operation of its business. Such activities may include,
but are not lited to, equipment or facilties additions, removal or rearrangements and
routie preventative maiteance. Generaly, such activities are not speciic to an
individual Customer but afect may Customers' services. No specifc advance notication
period is applicable to al servce activities. The Company wi work cooperatively witl the
Customer to determie the reasonable notication requirements. With some emergency or
unplaed service-afectig conditions, such as an outage resultig from cable damage,
notication to the Customer may not be possible.

2.1.6 Provision of Equipment and Facilties
(A) The Company shall use reasonable efforts to mainta only the facilties and
equipment that it furnihes to the Customer. The Customer may not nor may the
Customer permit others to rearrange, disconnect, remove, attempt to repair, or
otherwe interfere with any of the facilties or equipment intalled by the
Company, except upon the writtn consent of the Company.
(B) The Company may substitute, change or rearrange any equipment or facilty at any
tie and from tie to tie, but shal not thereby alter the techncal parameters of
the service provided by the Customer.
(C) Equipment the Company provides or intall at the Customer Premies for use in
connection with the services the Company offers shal not be used for any purpose
other than that for which the equipment is provided.
(D) Except as otherwe indicated, Customer provided station equipment at the
Customer's premies for use in connection with the service shal be so constrcted,
maintaed and operated as to work satifactorily with the facilties of the
Company.
(E) The Company shall not be responsible for the intaation, operation, or
mainteance of any Customer provided communcations equipment. Where such
equipment is connected to the facilties fuhed pursuant to th tari, theresponsibilty of the Company shal be lited to the furnihig of facilties offered
under th ta and to the maitenance and operation of such facilties. Subject to
th responsibilty, the Company shal not be responsible for:
(1) the though tranmision of signals by Customer provided equipment or
for the qualty of, or defects in, such tranmision; or
(2) the reception of sign by Customer-provided equipment; or
(3) network control signalg where such signalig is performed by Customer-
provided network control signalg equipment.

2.1.7 Non-Routine Installation
At the Customer's request, intaation and/ or maitenance may be performed outside the
Company's regular business hours or in hazardous locations. In such cases, charges based
on cost of the actual labor, mateia, or other costs incured by or charged to the Company
wi apply. If intaation is staed durg reguar business hours but, at the Customer's
request, extends beyond regur business hours into tie periods includig, but not lited
to, weekends, holidays, and/ or night hours, additional charges may apply.
2.1.8 Special Construction
Subject to the agreement of the Company and to al of the regulations contained in th
taif, special constrction or facilties may be undertaken on a reasonable efforts basis at the
request of the Customer. Special construction is construction undertaken:
(A) where facilties are not presetly avaiable, and there is no other requiement for the
facilties so constrcted;
(B) of a type other than that which the Company would normally uti in the
funihig of its servces;
(C) over a route otler than that which the Company would normaly utie in the
furnihig of its services;
(D) in a quantity greater than that which the company would normaly constrct;
(E) on an expedited basis;
(F) on a temporar basis unti perment facilties are available;
(G) involving abnormal costs; or
(H) in advance of its normal constrction.

2.1.9 Ownership of Facilities
Title to al facilties provided in accordance with th tarif remai in the Company, its
partners, agents, contractors or suppliers.
2.2 Prohibited Uses
2.2.1 The services the Company offers shal not be used for any unawfu purpose or for any use
as to which tle Customer has not obtained al required governmental approvals,
authoriztions, licenes, consents and permts.
2.2.2 The Company may requie applicants for service who intend to use the Company's
offerings for resale and/ or for shared use to fie a lettr witl the Company confmig that
their us of the Company's offerings complies witl relevant laws and the Idaho
Commsion's regulations, policies, orders, and decisions.
2.2.3 The Company may block any signal being tranmittd over its Network by Customers
which caus interference to tle Company or other users. Customer shal be relieved of all
obligations to make payments for charges relatig to any blocked Service and shall
indemn the Company for any clai, judgment or libilty resultig from such blockage.
2.2.4 A Customer, joint user, or authorizd usr may not assign, or tranfer in any maner, the
servce or any rights associated with the servce without tle wrtten consent of the
Company. The Company wil permit a Customer to tranfer its exitig service to another
entity if the existig Customer has paid all charges owed to the Company for regulated
communcations servces. Such a tranfer wi be treated as a diconnection of exitig
servce and intaation of new service, and non-recuring intaation charges as stated in
th taif wi apply.

2.3 Obligations of the Customer
2.3.1 General
The Customer shal be responsible for:
(A) the payment of al applicable charges pursuant to th taif;
(B) damage to or loss of the Company's facilties or equipment caused by the acts or
omisions of the Customer; or the noncompliance by the Customer, with these
reguations; or by fie or theft or other casualty on the Customer Premies, uness
caused by the negligence or wi miconduct of the employees or agents of the
Company;
(C) providing at no charge, as specifed from tie to tie by the Company, any needed
equipment, space and power to operate Company facilties and equipment
intaed on the premies of the Customer, and the level of heatig and air
conditionig necessar to maita the proper operatig environment on such
premis;
(D) obtag, maintain~ and otherwis having fu responsibilty for al rights-of-
way and conduit necessar for intaation of fiber optic cable and associated
equipment used to provide Communcations Services to the Customer from the
cable buidig entrance or propert lie to the location of the equipment space
described in Setion 2.3.1(Q. Any and al costs associted with the obtag and
maintaing the rights-of-way described herein, including the costs of altering the
strctue to permit intaation of the Company provided facilties, shall be borne
entiely by, or may be charged by the Company, to the Customer. The Company
may require the Customer to demonstrate its complice with th setion prior to
acceptig an order for service.

(E) providig a safe place to work and complying with all laws and reguations
regardig the workig conditions on the premies at which Company employees
and agents shal be intag or maintag the Company's facilties and
equipment. The Customer may be requied to intall and maintain Companyfacilties and equipment with a hazardous area if, in the Company's opinon,
injury or damage to the Company employees or property might result from
intallation or maitenace by the Company. The Customer shal be responsible
for identiing, monitoring, removing and diposing of any hazdous material
(e.g., friable asbestos) prior to any constrction or intaation work;
(F) complyig with al laws and reguations applicable to, and obtag all consents,
approvals, licenses and permts as may be requied with respect to, the location of
Company facilties and equipment in an Customer premies or the rights-of-way
for which Customer is responsible under Setion 2.3.1(D); and grantig or obtag
permision for Company agents or employees to enter the premies of the
Customer at any tie for the purose of intallg, inpectig, maitag,
repairg, or upon termition of service as stated herein, removing the facilties or
equipment of the Company;
(G) not creatig or alowig to be placed any lien or other encumbrances on the
Company's equipment or facilties; and
(H) makig Company facilties and equipment avaible periodicaly for maitenance
purposes at a tie agreeable to both the Company and the Customer. No
alowance wi be made for the period during which servce is interrupted for such
puroses.

2.3.2 Liabilty of the Customer
(A) The Customer wil be liable for damages to the facilties of the Company and for all
incidental and consequential damages caused by the negligent or intentional acts or
omisions of the Customer, its offcers, employees, agents, invites, or contractors
where such acts or omisions are not the diect result of the Company's negligence
or intentional miconduct.
(B) To the extent caused by any negligent or intentional act of the Customer as
described in (A), precedig, the Customer shall indemn, defend and hold
harmless the Company from and againt al cla, actions, damages, liabilties,
costs and expenses, includig reasonable attorneys' fees, for (1) any loss,
destruction or damage to property of any thd party, and (2) any liabilty incurred
by the Company to any thd par pursuant to th or any other tariff of the
Company, or otherwe, for any interrption of, interference to, or other defect in
any service provided by the Company to such tllld party.
(q The Customer shal not assert any clai agait any other Customer or user of the
Company's services for damages resultig in whole or in party from or arising in
connection with the fuhig of service under th ta includig but not lited
to mitaes, omisions, interruptions, delays, errors or other defects or
mirepresentations, whether or not such other Customer or user contributed in any
way to the occurence of the damages, uness such damages were caused solely by
the negligent to intentional act or omision of the other Customer or user and not
by any act or omision of the Company. Nothg in th taif is intended either to
lit or to expand Customer's right to assert any cla agait thd parties for
damages of any natue other than those descrbed in the precedig sentence.

2.4 Customer Equipment and Channels
2.4.1 General
A user may tranmit or receive inormation or signals via the facilties of the Company. The
Company's services are designted primarily for the tranmision of voice-grade telephonic
signals, except as otherwise stated in th tari. A user may tranmit any form of signal that
is compatible with the Company's equipment, but the Company does not guarantee that its
services wil be suitable for puroses other than voice-grade telephonic communcation
except as specifcally stated in th taif.
2.4.2 Station Equipment
(A) Termal equipment of the user's premies and the electric power consumed by
such equipment shal be provided by and maitaed at the expense of the user.
The user is responsible for the provision of wiing or cable to connect its termal
equipment to the Company MPP.
(B) The Customer is responsible for ensuring that Customer-provided equipment
connected to Company equipment and facilties is compatible with such equipment
and facilties. The magntude and character of the voltages and curents impressed
on Company-provided equipment and wiing by connection, operation, or
maitenance of such equipment and wig shal be such as not to caus damage to
the Company-provided equipment and wiing or injur to the Company's
employees or to other persons. Any additional protetive equipment requied to
prevent such damage or injur shal be provided by the Company at the
Customer's expense, subject to prior Customer approval of the equipment expee.

2.4.3 Interconnection of Facilities
(A) Local Traffic Exchange provides the abilty for another local exchange provider to
termiate local traffic on the Company's network. In order to quali for Local
Traffic Exchange the cal must (a) be origited by an end user of a company that is
authorized by the Commsion to provide local exchange service; (b) origiate and
termiate within a local callg area of the Company.
(B) Any special interface equipment necessaiy to achieve compatibilty between the
facilties and equipment of the Company used for fuhing Communcations
Servces and the chanels, facilties, or equipment of others shall be provided at the
Customer's expense.
(C) Communcations Services may be connected to the services or facilties of other
communications carriers only when authorized by, and in accordance with, the
terms and conditions of the taifs of the other communcations carriers which are
applicable to such connections.
(D) Facilties funihed under th ta may be connected to Customer provided
termial equipment in accordance with the proviions of th taif. Al such
termal equipment shal be regitered by the Federal Communcations
Commsion pursuant to Part 68 of Title 47, Code of Federal Reguations; and al
user-provided wiing shal be intaed and maitaed in complice with those
regutions.

2.4.4 Inspections
(A) Upon suitable notication to the Customer, and at a reasonable tie, the Company
may make such tests and inpections as may be necessary to determe llat the
Customer is complying with the requirements set forth in Setion 2.4.2(B) for the
intallation, operation, and maintenance of Customer-provided facilties,
equipment, and wiring in the connecton of Customer-provided facilties and
equipment to Company-owned facilties and equipment
(B) If the protective requiements for Customer-provided equipment are not being
complied with, the Company may take such action as it deems necessar to protect
its facilties, equipment, and personneL. The Company wil noti the Customer
promptly if there is any need for further corrective action. With ten days of
receiving th notice, the Customer must tae th corrective action and noti the
Company of the action taken. If the Customer fail to do th, the Company may
tae whatever additional action is deemed necessary, including the suspension of
service, to protect its facilties, equipment and personnel from harm.
(C) If harm to the Company's network, personnel or services is iment, the
Company reserves the right to shut down Customer's service imediately, will no
prior notice requied.

2.6 Payment Arrangements
2.6.1 Payment for Servces
The Customer is responsible for the payment of al charges for facilties and servIces
furnhed by the Company to the Customer.
The Customer is responsible for the payment of federal excise taxes, state and local sales
and use taes and simar taxes imposed by governmental jurisdictions, all of which shall be
separately designated on the Company's invoices. Any taxes imposed by a local jurisdiction
(e.g., county and muncipal) wil only be recovered from those Customers residing in the
afected jurisdictions.
2.6.2 Billng and Collection of Charges
The Customer is responsible for payment of all charges incurred by the Customer or other
users for services and facilties furnhed to the Customer by the Company.
(A) Non-recing charges are due and payable with thty (30) days afr the date
the invoice is mailed to the Customer by the Company.
(B) The Company shall present invoices for recurrig charges monthy to the
Customer, in advance of the month in which service is provided, and recurring
charges shal be due and payable with thty (30) days afer the date the invoice is
maed to the Customer by the Company. When bilg is based upon Customer
usage, usage charges wi be biled monthy for the precedig bilg period.
(C) When servce does not begi on the first day of the bilg period, or end of the last
day of the bilg period, the charge for the fraction of the month in which service
was furnihed wi be calcuated on a pro rata basis. For th purpose, every month
is considered to have thty (30) days.

(D) Bilg of the Customer by the Company wi begi on the Service Commencement
Date, which is tlie day on which the Company noties the Customer that the
service or facilty is avaiable for use, except that the Service Commencement Date
may be postponed by mutual agreement of the parties, or if the service or facilty
does not conform to stadards set fort in th tariff or the Service Order. Bilg
accrues fuough and includes the day that the service, circuit, arrangement or
component is dicontiued.
(E) If any porton of the payment is not received by tlie Company witlli 30 days of
receipt of th bil, or if any portion of the payment is received by the Company in
fuds which are not imediately availble upon presentment, then a late payment
charge of 1.5% per month shal be due to the Company. A late payment charge is
not applicable to subsequent rebilg of any amount to which a late payment
charge has already ben applied. Late payment charges are to be applied without
dicrimation.
(F) the Customer should noti the Company of any diputed items on an invoice
with sixty (60) days of receipt of the invoice. If the Customer and the Company
are unble to resolve the dipute to their mutual satifaction, the Customer may fie
a complait with the Commsion in accordance with the Commsion's rues and

The date of the dispute shal be the date the Company receives sufficient
documentation to enable it to investigate the dispute. The date of the resolution
is the date the Company completes its investigation and notiies the Customer of

(G) If servce is diconnected by the Company (in accordance with Section 2.6.3followig) and later re-intaed, re-intation of servce wi be subject to all
applicable intaation charges. If servce is suspended by the Company (in
accordance with Setion 2.6.3 followig) and later restored, restoration of service
wi be subject to the rates in Setion 4.3.

2.6.3 Discontinuance of Service for Cause
The Company may dicontiue service for the followig reasons provided in th Section
2.6.3. Customers wil be provided ten (10) days written notice prior to discontiuance
uness otherwise indicated. Notice wi be provided via First Class U.S. Mail.
Upon the Company's dicontiuance of service to the Customer under Section 2.6.3(A) or
2.6.3(B), the Company, in addition to al other remedies tht may be available to the
Company at law or in equity or under any other provision of th taiff, may declare al
futue monthy and other charges which would have been payable by the Customer during
the remainder of the term for which such servces would have otherwise been provided to
the Customer to be imediately due and payable.
(A) Upon nonpayment of any amounts owing to the Company, the Company maydiscontiue or suspend servce without incuring any liabilty. Company wil

(B) Upon violation of any of the other material terms or conditions for furnhig
service the Company may, diontiue or suspend servce without incurring any
libilty if such violation contiues during that period.
(C) Upon condemntion of any material portion of the facities used by the Company
to provide servce to a Customer or if a casualty renders al or any material portion
of such facilties inoperable beyond feasible repai, the Company, by notice to the
Customer, may dicontiue or suspend servce without incurig any liabilty.
(D) Upon the Customer's inolvency, assignent for the benefit of creditors, fig forbanruptc or reorganation, or failg to dicharge an involunta petition with
the tie permttd by law, the Company may imediately dicontiue or suspend
service without incurig any liabilty. Company wil comply with Federal
Bankruptcy law and rules.
(E) Upon any governenta prohibition or governental requied alteration of the
servces to be provided or any violation of an applicable law or reguation, the
Company may imeditely diontiue servce without incurig any liabilty.

2.6.4 Notice to Company for Cancellation of Servce
Customers desiring to termiate service shal provide the Company thty (30) days notice
of desire to termiate service. If special constrction is involved, the required notice shal be
written.
2.6.5 Cancellation of Application for Servce
(A) Where the Company permts the Customer to cancel an application for service prior
to the start of service or prior to any special constrction, no charges wi be imposed
except for those specifed below.
(B) Where, prior to cancellation by the Customer, the Company incus any expenses in
intag the service or in preparing to inta the service that it otherwise would not
have incured, a charge equal to the costs the Company incurred, less net salvage,
shal apply, but in no case shal th charge exceed the sum of the charge for the
mium period of servces ordered, includig intallation charges, and all charges
others levy agait the Company that would have ben chargeable to the Customer
had servce begu.
(C) Where the Company incurs any expense in connection with special constrction, or
where specil arangements of facilties or equipment have begu, beore the
Company receives a cancelltion notice, a charge equal to the costs incured, less net
salvage, may apply. In such cases, the charge wi be based on such elements as the
cost of the equipment, facities, and material, the cost of intallation, engieering,
labor, and supervision, general and admtrative expense, other dibursements,
depreciation, maitenance, taxes, provision for return on investment, and any other
costs associated with the special constrction or arrangements.
(D) The special charges described in 2.6.5(A) though 2.6.5(C) wil be calcuted and
applied on a case-by-ease basis.

2.6.6 Changes in Servces Requested
If the Customer makes or requests material changes in circuit engieering, equipment
specifications, servce parameters, premies locations, or otherwe materialy modies any
provision of the application for service, the Customer's intallation fee shall be adjusted
accordigly.
2.6.7 Bad Check Charge
A service charge of $20.00 wil be assessed for all checks retued by a ban or other
fiancial intitution for: Inufcient or uncollected fuds, closed account, apparent
tampering, mising signture or endorsement, or any other inufciency or dicrepancy
necessitatig return of the intrment at the dicretion of the drawee ban or other fiancial
intitution.
2.7 Allowances for Interrptions in Servce
2.7.1 General
Credit for Interruptions: When the use of service or facilties furnihed by the Company is
interrupted due to any cause other than the negligence or wil act of the Customer, or
the operation or failure of the facilties or equipment provided by the Customer, pursuant
to IDAPA 31.41.01 Riles 502 and 503 subject to interruption wil be allowed for the
service and facilties rendered useless and inoperative by reason of the interruption,
whenever said interruption contiues for a period of 24 hours or more from the tie the
interruption is reported to or known to exist by the Company, except as otherwise
specifed in the Company's tariffs. If the Customer reports a service, facilty or circut to
be inoperative but declies to release it for testig and repai, it is considered to be
impaired, but not interrupted.
For calculatig credit allowances, every month is considered to have 30 days. A credit
allowance is applied pursuant to IDAP A 31.41.01 Rules 502 and 503 based on the rates
specified hereunder for Local Lie or Local Tru Service and is dependent upon the
length of the interruption. Only those facilties on the interrupted portion of the circuit
wil receive a credit. Credit alowances for service outages that exceed 24 hours in
duration wi be rounded up at the next whole 24 hours.

2.7.1 General (Continued)
(C) If the Customer reports a servce, facilty or circuit to be interrupted but declies to
release it for testig and repai, or refues access to its premies for test and repair by
the Company, the service, facilty or circuit is considered to be impaied but not
interrupted. No credit alowances wi be made for a service, facilty or circuit
considered by the Company to be impaied.
(D) The Customer shall be responsible for the payment of service charges as set forth
herein for visits by the Companý s agents or employees to the premies of the
Customer when the service dicuty or trouble report results from the use of
equipment or facilties provided by any party other than the Company, including but
not lited to the Customer.
2.7.2 Limitations of Alowances
No credit alowance wil be made for any interruption in service:
(A) interruptions due to the negligence of, or noncompliance with the provisions of th
taiff by, the Customer, Authorid User, Joint-User, or other common carrier
providing service connected to the service of Company;
(B) interruptions due to the negligence of any person other than the Company
includig, but not lited to, the Customer connected to the Company's facilties;
(C) interruptions due to the failure or malction of non-Company equipment;
(D) interruptions of service durig any period in which the Company is not given full
and free access to its facilties and equipment for the purpose of investigatig and
correctig interruptions;

(E) interruptions of service during a period in which the Customer contiues to use the
service on an impaired basis;
(F) interruptions of service during any period when the Customer has released service
to the Company for maintenance purposes or for implementation of a Customer
order for a change in service arrangements;
(G) interruption of service due to circumstaces or causes beyond the control of the
Company.
2.7.3 Use of Another Means of Communications
If the Customer elects to use another mean of communcations during the period of
interruption, the Customer must pay the charges for the alterntive service used.
2.8 Cancellation of Servcelermnation Liabilty
If a Customer cancels a service order or termite services before the completion of the tem for any
reason other than a servce interrption (as defied in Seon 2.7.1) or where the Company breaches
the terms in the service contract, Customer may be requested by the Company to pay to Company
termiation libilty charges, which are defied below. These charges shal beome due and owig
as of the effective date of the cancellation or termiation and be payable with the period set forth
in Setion 2.6.2.
2.8.1 Termination Liabilty
Customer's termation liabilty for cancellation of servce shal be equal to:
(A) al unpaid non-recurring charges reasonably expended by Company to establih
service to Customer, plus;
(B) any disconnection, early cancellation or termiation charges reaonably incued and
paid to thd parties by Company on behal of Customer, plus;
(C) all recuring charges specifed in the applicable Service Order for the balance of the
then current term disounted at the prime rate anounced in the Wall Street Journ on
the thd business day followig the date of cancellation;
(D) minus a reasonable allowance for costs avoided by the Company as a diec reult of
Customer's cancellation.

2.9 Transfers and Assignments
Neitlier tlie Company nor the Customer may assign or tranfer its rights or duties in connection with
the services and facilties provided by the Company without the written consent of the other party,
except that the Company may assign its rights an duties to a) any subsidiary, parent company or
afilate of the Company; b) pursuant to any sale or tranfer of substatially al the assets of the
Company; or c) pursuant to any fiancing, merger or reorganation of the Company.
2.10 Flexible Pricing
Notice to Customers of rate changes shall be made in accordance with Idaho Code 62-606. Where
there are no regulations, notiication wil be made TIl a maner appropriate to the circumstances
involved. A Customer can request that the Company disconnect service that is provided under
the Flexible Pricing due to a price increase. The customer wil be credited for the difference
between the new price and the old price retroactive to the effective date of the price increase if the
customer noties the Company of its desire to disconnect service with 20 days of receiving
notiication of the price increase.

2.12.2 The Customer shall designate on tlie service order an address to which the Company shal
mail or deliver all notices and other communcations, except that Customer may also
designate a separate address to which the Company's bil for service shall be maied.
2.12.3 The Company shall designate on the service order an address to which the Customer shal

a separate address on each bil for service to which the Customer shall mail payment on

2.12.4 Except as otherwise stated in th tarif, all notices or other communications required to be

the other party on the third business day following placement of the notice,
communcation or bil with the U.S. Mail or a private delivery servce, prepaid and
properly addressed, or when actualy received or refued by the addressee, whichever

3.2 Rate Classes
Charges for local services provided by the Company may be based, in part, on the Rate Class
associated with the Customers End Office. The Rate Class is determied by the total access lies
and PBX tr in the local callg area which can be reached from each End Office.
In the event that an Incumbent LEC or the Idaho Commsion reclassifes an exchange from one Rate
Class to another, the reclassifcation wi alo apply to customers who purchase servces under th

5.2 Call Timing for Usage Sensitive Services
Where charges for a service are specified based on tlie duration of use, such as the duration of a
telephone call, the following rules apply:
5.2.1 Call are measured in durational increments identied for each service. Al call, which are
fractions of a measurement increment, are rounded-up to the next whole unt.
5.2.2 Timg on completed cal begi when the call is anwered by the called party. Answering
is determied by hardware anwer supervision in al cases where th signalg is provided
by the termitig local carrier and any intermedite carrier(s).
5.2.3 Timg termates on al call when the calg party hangs up or the Company's network
receives an off-hook signal from the termiatig carrier.

5.3 Distance Calculations
Where charges for a service are specifed based upon distace, the following rules apply:
5.3.1 Distance between two points is measured as airlie distace between the rate centers of the
origiatig and termiatig telephone lines. The rate center is a set of geographic
coordiates, as referenced in Local Exchange Routig Guide issued by Bellcore, associated
with each NPA-NXX combination (where NP A is the area code and NX is the fist fuee
digits of a seven-digit telephone number). Where there is not telephone number associated
with an access lie on the Company's network (such as a dedicated 800 or WATS access
line), the Company wil apply the rate center of the Customer's mai bilg telephone
number.
5.3.2 The ailie distace between any two rate centers is determied as follows:
Step 1: Obtain the "V" (vertical) and "H" (horizonta) coordiates for each Rate Center
from the above-referenced Bellcore docent.
Step 2: Computer the diference between he "V" coordinate of the two rate centers; and
the dierence between the two "H" coordinates.
Step 3: Square each dierence obtaed in step (b) above.
Step 4: Add the square of the "V" dierence and the square of the "H" dierence
obtaed in step C) above.
Step 5: Divide the sum of the squares by 10. Round to the next higher whole number if
any fraction is obtaed.
Step 6: Obtain the square root of the whole number result obtaed above. Round to the

5.5 Standard Residence Line
The Standard Residence Line provides a Customer Witli a single, analog, voice-grade telephonic
communcations chanel, which can be used to place or receive one call at a tie. Stadard
Residence Lines are provided for the connection of Customer-provided wig and single station
sets or facsime machines.
5.6 Standard Business Line
The Stadard Business Line provides a Customer with a single, anog, voice-grade telephonic
communcations chanel, which can be used to place or receive one cal at a tie. Stadard
Business Lines are provided for the connection of Customer""provided wiing and single station sets
or facsime machies. An optional per lie Huntig featue is available for multi-lie Customers
which routes a call to an idle station lie in a prearranged group when the caled station lie is busy.
5.7 PBX Trunk Servce
Basic PBX Trun Service provides a Customer with a single, voice-grade telephonic communcations
channel, which can be used to place or receive one call at a tie. Basic Tru are provided for
connection of Customer-provided private branch exchanges (PBX) to the public switched
telecommuncations network. Each Basic PBX Trun is provided with touch-tone signalg and may
be configued into a hunt group at no additional charge with other Company-provided Basic PBX
Tru. The signal is an analog signal at the DSO leveL.
5.8 Reserved for Futue Use
5.9 Direct Inward Dialing (DID) Servce
Direct Inward Dialig ("DID") permts cal incomig to a PBX system or other Customer Premies
Equipment to be routed to a specifc station without the assistace of an attendant. DID call arerouted diectly to the station associated with the called number. DID service as offered by the
Company provides the necessary trun, telephone numbers, and out-puling of digits to enables
DID service at a Customet s location. DID service requires special PBX softare and hardware not
provided by the Compary. Such hardware and software is the responsibilty of the Customer.

5.11 Optional Callng Features
The features lited in Setion 5.11.1 are offered by the Company to Business Customers. Refer to
Price Lists in Setion 7 of th tarif for speifc featues offered with each type of local exchange
servce.
5.11.1 Featues Descriptions
(A) Flexible Call Forwarding: Provides end-user control for cal forwarding capabilties via
dial-accessed voice prompt menus. Customers may forward cal to a primry local or
long distace. The end-user may specif a secondary location for routig of go
unanwered at the forward-to location or reach a busy sign. Th secondary location
may be another telephone number, pager or voice messagig service. Other capabilties
included with th feature include:
Speed Forwardig;
Priority Sceenig;
Rig Control; and
Timed Forwarding.
It is the responsibilty of the Customer to subscribe to the telephone number, pager or
voice messagig servce used as the secondar location.
(B) Call Forwarding Don't Answer: Permts the forwardig of incomig cal when theend-user's lie remai unwered afr a pre-designated rigig interval. The
ringig interval before forwardig and the forward-to number are fied by the service
order. However, the end-user has the abilty to turn the featue on or off at hi/her
discretion.
(C) Call Forwarding Busy Line: Permits the forwardig of incomig call when the end-
user's lie is busy. The forwarded number is fied by the end-user service order.
However, the end-usr has the abilty to turn the feature on or off at hijher discretion.

(D) Preferred Call Forwarding: Permits the end-user to automatically forward to another
number call received from up to six end-user pre-selected telephone numbers
programmed into the featues screenig lit. The end-user controls when the featue is
active, the forward-to-number and can add or remove calg numbers from the
feature's screenig lit.
(E) Call Forwarding Variable: Permits the end-user to automaticaly forward (transfer) allincomig cal to another telephone number, and to restore it to normal operation at
their discretion. The end-user must dial an activation code from hijher exchange lie
along with the forward-to number in order to turn the featue on. A separate code is
dialed by the end-user to deactivate the feature.
(F) Cal Forwarding Variable, Remote Access: Permits the end-user to automaticaly
forward (tranfer) all incomig cal to another telephone number, and to restore it to
normal operation at their dicretion. The end-user must dial an activation code along
with the forward-to number in order to turn the feature on. A separate code is diaed
by the end-user to deactivate the featue. Featue activation may be performed from
the end-user's exclge lie or remotely from some other lie. Remote access requies
the end-user to (1) dial a speial access number 2) enter their seven-digit telephone
number and 3) enter a personal identication number prior to forwardig their call.
(G) Three Way Calling: Permts the end-user to add a thd par to an establihed
connection. When the thd party anwers, a two-way conversation can be held before
addig the origial party for a theeway conference. The end-user intitig the
conference controls the cal and may disconnect the thd party to reestablih the
origil connection or establih a connection to a dierent thd party. The feature may
be used on both outgoing and incomig cal.
(H) Multiple Directory Number Distinctive Riging: Th feature allows an end user to
determie the source of an incomig call from a distictive ring. The end user may
have up to two additional numbers assigned to a single lie (i.e. Distictive Rigig-
First Number and Distictive Rigig - Seond Number). The designated primary
number wil receive a normal ringig pattrn; other numbers wi receive ditictive
ringig pattern. The pattrn is based on the telephone number that the calg party
dials.

(I) Speed Caling: Permits the Customer to place call to other telephone numbers by
dialg a one or two digit code rather than the complete telephone number. The featue
is avaiable as either an eight (8) code lit or a thirty (30) code lit Code lits may
include local andj or toll telephone numbers. The Customer has the abilty to add or
remove telephone numbers and codes tojfrom the speed calg lit without assistace
from the Company.
m Repeat Dialing: Permits the end-user to have call automatically redialed when the fist
attempt reaches a busy number. The lie is checked every 45 seconds for up to 30
miutes and alerts the Customer with a distictive ringig pattern when the busy
number and the Customer's lie are free. The Customer can contiue to make and
receive cal whie the featue is activated. The following types of cal canot be
reached using Repeat Dialig:
Cal to 800 Service numbers
Cal to 900 Servce numbers
Cal preceded by an interexchange carrier access code
Interntiona Direct Distace Diaed cal
Cal to Directory Assistace
Cal to 911
(K) Caller ID: Permits the end-user to view a Direcory Name and Directory Number of thecalg party on incomig telephone cal. Inormation is displayed on a speciad
CPE not provided by the Company. The feature alo provides the date and tie of
each incomig cal. It is the responsibilty of the Customer to provide the necessary
CPE. In some situations, the calg partys city and state may be displayed rather than
a Directory Name, depending on available call data.
(L) Call Selector: Allows a Customer to assign a maximum of 15 telephone numbers to a
special lit. The Customer wil hear a distictive ring when cal are received from
telephone numbers on that lit

(M) Call Waiting: Allows the end-user to control the treatment applied to incomig cal
whie the Customer is off-hook on an existig call. Th featue includes the
capabilties of Cal Waitig Basic plus additional cal treatment options. Treatment
options offered with Call Waitig Deluxe include:
Anwer the waitig call and placing the fist party on hold;
Anwer the waitig cal and disconnectig from the first party;
Direct the waitig caler to hold via a recordig
Forward the waitig caller to another location (e.g., voice mailbox or telephone
anwering service)
Ful utiation of Call Waitig Deluxe requies specialied CPE not provided by the
Company. It is the responsibilty of the Customer to provide the necessary CPE. The
end-user must have Caller il Basic or Deluxe for diplay of callg party identication
inormation for waitig cal. The end-user must have a Cal Forwardig dont
Anwer featue active in order to forward a waitig cal to another location.
(N) Cal Tracing: Alows the tracig of nuiance cal to a specifed telephone numbersuspected of origitig from a given local office. The tracig is activated upon
enterig the speifed dial code. The origiatig telephone number, outgoing tr
number or termiatig number, and the tie and date are generated for every call to
the specifed telephone number can then be identied.
(0) Call Block: Allows the end-user to automaticaly block incomig call from up to six
end-user pre-selected telephone numbers programed into the feature's screenig lit.
Calers whose numbers have ben blocked wi hear a recorded message statig tht
their call has been blocked. The end-user controls when the featue is active, and can
add or remove callg numbers from the feature's screenig lit.

(P) Call Retun: Alows the Customer to return a call to tlie last incoming call whether
anwered or not. Upon activation, it wil redial the number automatically and contiue
to check the number every 45 seconds for up to 30 minutes if the number is busy. The
Customer is alerted with a ditictive rigig pattern when the busy number is free.
When the Customer anwers the rig, the cal is then completed. The calg parts
number wi not be delivered or anounced to the cal recipient under anycicutaces.
(Q) Anonymous Cal Rejection: Permits the end-user to automatically reject incomig call
when the call origiates from a telephone number which has blocked delivery of its
callg number (see Callg Number Delivery Blockig). When active, call from
private numbers wil be routed to a special anouncement then termiated. The feature
may be tued on or off by the end-user by dialg the appropriate featue control code.
Anonymous Ca Rejection is offered as a stad-alone feature or as an add-on to Caller
IDDeluxe.
(R) Calling Number Delivery Blockig: Prevents the delivery, diplay and anouncement
of the end-user's Directory Number and Direcory Name on all cal dialed from an
exchange servce equipped with th option. When active, the end-user's telephone
name and number wi not appear on the caled partys Caler ID CPE or be dislosed in
another way. The feature is availble on a per cal or per lie basis. With per cal
Calg Number Delivery Blockig, it is necessary for the end-user to dial an activation
code prior to placing the cal. With the per lie version of the featue, al call are
placed with the end-user's number blocked. Per lie end-users must dial an activation
code prior to utiation.
(S) Message Waiting Indication: Provides the end-user with an audible (stutter dial tone)or viual (lamp or other CPE display) indication that messages are waitig to be
retrieved. Message Waitig Indication can only be actvatedj deactivated by a voice
mailbox or other voice messagig service provided by the Company or thd party. It is
the responsibilty of the Customer to subscribe to a compatible voice messagig service.
Visual Message Waitig Indication requires specd CPE not provided by the
Company. It is the responsibilty of the Customer to provide the necessary CPE.

5.12 Listing Services
For each Customer of Company-provided Exchange Service(s), the Company shall arrange for the
litig of the Customer's mai bilg telephone number in the directory(ies) publihed by the
domiant Local Exchange Carrier in the area at no additional charge. At a Customer's option, the
Company wil arrange for additional litigs for an additional charge.
5.12.1 Non-Published Service
Th optional service provides for suppression of printed and recorded directory litigs. A
Customer's name and number do not appear in printed directories or Directory Assistace
Bureau records.
5.12.2 Non-Listed Servce
Th optional service provides for suppression of printed diecory litigs only. Parties
may sti obta the Customer's number by calg the Directory Assistace Bureau.
5.13 Directory Assistance
Provides for identication of telephone diectory numbers, via an operator or automated platform.
Customers are provided with a maximum of 2 litigs per each cal to Directory Assistance.
5.14 Miscellaneous Servces
5.14.1 Presubscrption Servces
Th service provides for the Presubscription of local exchange lies provided by the
Company to the intraLATA and interLATA long ditace carrier(s) selected by the
Customer.
5.14.2 Pay Per Cal Blockig/nblockig
Th servce provides the option of blockig, or subsequent unblockig, all 900 and 976 cal
on a per lie basis. The Company wi provide for per-lie blocking where the Companýs
switchg facilties permit.

specifed local exchange services to Customers tlough resale of local exchange services.
Al rates set forth in th Setion are subject to change and may be changed by the Company
pursuant to notice requirements establihed by the Idaho Commsion. The rate, terms and
conditions set forth in th Setion are applicable as of the effective date hereof and wi not apply to
any Customer whose servces may have ben provisioned tlough resale of the Company's local

7.3.2 Hunting (afa Rotary or Grouping)
A. Description
Th is an optional arrangement avaiable to customers with two or more individual
lie or tr services. Where facilties permit, such lies/trun wi be arranged so
that incomig cal to a busy lie/trunk wil overflow to other available
lies/tr for that customer. The following types of huntig arrangements are
avaible: series and multie (basic huntig), circular and preferential.
B. Rates and Charges
1. The rate for each individual lie/ trun arranged for Huntig Service is in
addition to the reguar access lie/tr rate.
2. The nonrecurring charge applies for business customers to establih,
change to or from or to rearrange Huntig Service, except when changig

7.5 Business PBX Trunk Servce
PBX Trun service provides a Customer with a single, voice-grade telephonic communcations
chanel which can be used to place one cal at a tie. Tru are provided for connection of
Customer-provided private branch exchanges (PBX) or other station equipment to the public
switched telecommuncations network.
PBX Tru are available to Business Customers as Inward, Outward or Two-Way combination
trun where services and facilties permit.
Each PBX Tru is provided with Touchtone signalg at no additional charge. An optional per
tr Huntig feature is avaible for Customers which routes a call to the next idle trun in a
prearranged group (see Section 7.3).
PBX Trun may alo be equipped with Direct Inward Dialg (DID) capabilty and DID number
blocks for additional charges (see Setion 7.6). Usage charges are lited in Section 7.3.

7.6 Direct Inward Dialing (DID) Servce
Direct Inward Dialg ("DID") permits call incomig to a PBX system or other Customer Premies
Equipment to be routed to a specifc station without the assistace of an attendant. DID call are
routed diectly to the station associated with the caled number. DID service as offered by the
Company provides the necessar trun, telephone numbers, and out-pulsing of digits to enable
DID service at a Customer's location. DID service requies special PBX software and hardware not
provided by the Company. Such hardware and softare is the responsibilty of the Customer.
The followig charges apply to Customers subscribing to DID service provided by the Company.
These charges are in addition to recurring and non-recurring charges for PBX Tru as shown in
Section 7.5 of th taif. The Customer wil be charged for the number of DID numbers utid out
of the avaiable 20 numbers.

SECTION 9.0 - ADVANCED SERVICES
9.1 ISDN PRI Service with Unlimited Local Callng
ISDN PRI offers an array of value-added features, such as calling number identiication and call-
by-call selection that enhance productivity. ISDN PRI is configured with 23 64 Kbps bi-directional
B (Bearer) channels and one 64 Kbps D (Data) channeL. Unique to ISDN PRI is its abilty to
designate the D chanel to handle all of the signalig and call control requirements and leave the
remaig 23 B chanels free for any mix of circuit-switched voice and data.
Each of these products is offered under a 12, 24 or 36 month term agreement. Rates include
unlted local callg for sent-paid, directly dialed call. Rates do not include callg card call,
inormation type call to Time and Weather, 555, 700, 900, 976 Services, Directory Assistance or
any other type of Operator Handled call.
ISDN PRI includes the following non-optional Feature Package: Inbound Calling Line ID-Name &
Number and Call by Call Selection.
Regional Toll and Long Distance Services must be PIC'd to the Company. These rates are in
addition to ISDN PRI and DS1 rates below.

9.2 Digital DS-1 PBX Service with Unlimited Local Callng
This service provides a trunk side DS1 electrical interface from the customer's digital PBX system
to a digital port on a local Company switch for the origination and termination of call. Traffic to
and from the digital PBX can be received or dialed directly from any PBX station without the
need for an attendant.
These digital trun deliver a high-speed DS1 (Tl) connection between your PBX and the
Company network. There are up to 24 chanels on one facilty, each of which can be used to place
or receive call. Th multi-channel capabilty dramatically reduces the need for additional PBX
circuit cards.
Each of these products is offered under a 12, 24 or 36 month term agreement. Rates include
united local callg for sent-paid, directly dialed call. Rates do not include callg card call,
inormation tye call to Time and Weather, 555, 700,900,976 Services, Directory Assistace or
any other type of Operator Handled cal.
Regional Toll and Long Distance Services must be PIC'd to the Company. These rates are in
addition to ISDN PRI and DS1 rates below.

9.3 ISDN PRI Service with Unlimited Local Callng and Bundled TolJjD Service
ISDN PRI offers an array of value-added features, such as callg number identiication and call-
by-call selection that enhance productivity. ISDN PRI is configured with 23 64 Kbps bi-directional
B (Bearer) channels and one 64 Kbps D (Data) chaneL. Unique to ISDN PRI is its abilty to
designate the D channel to handle al of the signalig and call control requirements and leave the
remaing 23 B channels free for any mi of circuit-switched voice and data.
This product is offered under a 12, 24 or 36 month term agreement. Rates include unted local
callg for sent-paid, directly dialed call. Rates do not include calg card call, inormation tye

9.5 ISDN BRI Service
ISDN BRI (Basic Rate Interface) uses stadard "twisted pair" cables and is nearly three times
faster than a 56K dial up lie. ISDN PRI (Primar Rate Interface) uses a 1.544 Mbps digital
transport facility (Tl). Both services provide the superior clarity of digital transmission, a high-
speed data interface and sufficient bandwidth capacity to fulfil your current and future
communication needs.
ISDN BRI consists of two 64 Kbps B (Bearer) chanels and one 16 Kbps D (Data) channeL. Each B
chaIUlel has the ability to integrate voice, data, image and video. The B chanels may be kept
separate or bonded together to deliver 128 Kbps.

11.1 Carrier Presubscription

Carrier Presubscription is a procedure whereby a Customer designates to tlie Company
the carrier which the Customer wishes to be the carrier of choice for intraLAT A and
interLATA toll calls, Such cal are automatically directed to the designated carrier,
without the need to use carrier access codes or additional dialig to direct the call to the
designated carrier. Presubscription does not prevent a Customer who has presubscribed
to an IntraLATA or InterLATA toll carrier from using carrier access codes or additional
dialig to direct call to an alternative long distance carrier on a per cal basis.
11.1.2 Pre subscription Options - Customers may select the same carrier or separate carriers for
intraLATA and interLATA long distance. The following options for long distance

Customer may select two different carriers, neither being the Company
for intraLATA and interLATA toll call. One carrier to be the Customers'

Customer may select a carrier other than the Company for no
presubscribed carrier for intraLATA toll call subject to presubscription
which wil require the Customer to dial a carrier access code to route all

11.1.3 Rules and Regulations
Customers of record wi reta their primary interexchange carrier(s) unti they request tliat

Customers of record or new Customers may select either Options A, B, C, D, E or F for
intraLA T A Presubscription.
Customers may change their selected Option and/ or presubscribed toll carrier at any tie
subject to charges specifed in 11.1.5 below:
11.1.4 Presubscription Procedures
A new Customer wil be asked to select intraLATA and interLATA toll carriers at the tie
the Customer places an order to establih local exchange servce with the Company. The
Company wi process the Customer's order for service. All new Customers intial requests
for intraLA T A toll service presubscription shall be provided free of charge.
If a new Customer is unble to mae selection at the tie the new Customer places an order
to establih local exchange servce, the Company wi read a random litig of al avaiable

Unti the Customer inorms the Company of hi/her choice of primary toll carrier, the
Customer wi not have access to long ditace services on a presubscribed basis, but rather
wi be requied to dil a carrier access code to route all toll cal to the carier(s) of choice.
Customers who inorm the Company of a choice for toll carrier presubsciption with the
90-day period wi not be assessed a service charge for the intial Customer request.
Customers of record may intite an intraLATA or interLATA presubscription change at
any tie, subject to the chages speced in 11.1.5 below. If a Customer of record inquires
of the Company of the carriers avaiable for toll presubscription, the Company wi read a
random litig of al avaiable intraLATA carriers to aid the Customer in selection. ,

13.1 Promotional Offerings
The Company, upon notiication to Commssionr may make promotional offerings to its service
wlúch may include waiving or reducing the applicable charges for the promoted service. The
promotional offerings may be lited as to the duration, tlie date and ties of tlie offering and
the locations where the offerings are made.
13.2 Contract Rates j Individual Case Basis (ICB) Arangements
Competitive pricing arrangements at negotiated rates may be furnished on an individual case
basis (ICB) in response to request by customers to Unite Private Networks, for proposals or for
competitive bids. Servce offered under this tarif provision wil be provided to the Cu,tomer
pursuant to contract. Unless otherwise specifed, the reguations for such arrangements are i,
addition to the applicable regutions and prices in other sections of tlûs tarif. Specialied rates
or charges wil be made available to similarly situated Customers on a non-discriminatory basis.
Company wil provide the Commsion vvith copies of any ICB contractual arrangements.
