Check Sheets. When a tariff (price sheet) filing is made with the
Commission, an updated Check Sheet accompanies the filing. The Check
Sheet lists the sheets contained in the filing with a cross reference to the
current revision number. When new pages are added, the check sheet is
changed to reflect the revision.
Numbering (An example). Sheet numbers appear in the upper left corner
of each page. Sheets are numbered sequentially. When a new sheet is
added between sheets already in effect, a decimal is added. For example
a new sheet added between sheets 6 and 7 would be 6.
Sheet revision numbers also appear in the upper left corner of each page.
These numbers are used to determine the most current sheet version on
file with the the Commission. For example, the 4th revised sheet 6 cancels
the 3rd revised sheet 6. Consult the check sheet for the sheet currently in
effect.

Service will be offered throughout the State of Idaho.
A map of the state showing the entire area of service is attached hereto.
Deposit Requirement - No Deposit Required.
Termination of Service
The Company may deny or terminate a service or all services without prior notice
to the Customer or applicant and with the Customer s or applicant's permission for
one or more of the following reasons:
A condition immediately dangerous or hazardous to life, physical safety, or
property exists, or it is necessary to prevent a violation of federal, state or
local safety or health codes.
The Company is ordered to terminate service by any court, the
Commission , or any other duly authorized public authority.
The service(s) was (were) obtained, diverted or used without the
authorization or knowledge of the Company.

The Company has tried diligently to meet the notice requirements of the
Commission s Rule 304, but has been unsuccessful in its attempt to contact
the Customer affected.
The Customer has misrepresented the Customer s identity for purposes of
obtaining telephone service and has an outstanding bill exceeding one
hundred ($100) dollars.
Service Offering
The Company s services are offered to subscribers on a monthly basis.
1 The Company s services are offered twenty-four hours a day.
2 The Company s service agreement is in effect for a minimum of thirty days.
Liability .
The included tariff language does not constitute a determination by the
Commission that a limitation of liability imposed by the Company should be
upheld in a court of law. Acceptance for filing by the Commission
recognizes that it is a court's responsibility to adjudicate negligence and
consequential damage claims. It is also the court'responsibility to
determine the validity of the exculpatory clause.
The Customer will be liable for damages to the facilities of the Company
caused by negligence or willful acts of its officers, employees, agents or
contractors of the Customer.
The Company will give at least ten (10) days notice to Customers and the
Commission before increasing rates or other changes. The notice to
Customers will be either individual notice or public notice in the
newspapers in the Company s Idaho service areas.

UNI-NET BUSINESS AND BUSINESS 800

Uni-Net Business is a switched outbound and/or inbound access service designed for
small to medium size business applications. $50.00 minimum monthly expected usage
is required. Billing is with an eighteen (18) second minimum billing period and one-tenth
(1/1Oth) minute or six (6) second increments thereafter, at the per-minute rate specified
above.

UNI-NET ADVANTAGE AND ADVANTAGE 800

$0.01 for each minute of shortfall.

is a minimum requirement per T.1 of 150,000 minutes per month. Customer will be billed
