reflect the new name and reported that an Amendment of Registration of Foreign Limited
Liability Company is on file with the Idaho Secretary of State’s office.
IT IS HEREBY CERTIFIED that this Second Amended Certificate of Public
Convenience and Necessity,Certificate No.377,is issued to Level 3 Telecom of Idaho,LLC.
This Certificate authorizes Level 3 to provide resold and facilities-based basic local exchange
services within the State of Idaho.The Commission approved this name change to Certificate
No.377 by Order No.33537 in Case No.TWT-T-00-01.
