NOTICE OF T-MOBILE WEST CORPORATION
OF IMPENDING NAME CHANGE
T-Mobile West Corporation, wholly owned subsidiary of T-Mobile USA, Inc. and doing
business as T-Mobile (referred to as "T-Mobile" or "Company"), hereby provides the Idaho
Public Utilities Commission ("Commission") with notice that, effective June 25, 2012,
T-Mobile West Corporation will convert to and become T-Mobile West LLC. This change in
corporate organization will have no impact on the operations ofT-Mobile in the state of Idaho
and will no way impact T-Mobile's commitments to abide by the federal and state laws and

regulations impacting its designation as an Eligible Telecommunications Carrier ("ETC") in the
state of Idaho.
On August 9, 2011, this Commission issued Order No. 32319 in this Docket in which it
designated T-Mobile an ETC for certain areas in Idaho. On April 6, 2012, T-Mobile filed an
application in this Docket seeking conditional ETC designation for eligible census blocks outside
of its existing Idaho ETC area; such that T-Mobile shall be deemed an ETC outside its existing
ETC area where it is selected to receive Mobility Fund Phase 1 support following the Mobility
Fund Phase 1 auction, known as Auction 901. On May 3, 2012, the Commission issued Order
No. 32543 in this Docket in which it directed that T-Mobile's application for conditional ETC
designation be processed under Modified Procedure and established a 21-day comments period.
On May 24, 2012, Commission Staff filed comments recommending approval of T-Mobile's
application. Two additional comments were submitted by members of the public, both in
support of T-Mobile's application.
T-Mobile requests that the Commission acknowledge T-Mobile's impending conversion
and name change in its Order granting conditional ETC designation. T-Mobile also requests that
the Commission's Order acknowledge that the new name also applies to the previously issued
ETC designation granted in Order No. 32319. Given the timing of the corporate reorganization
resulting in a name change effective June 25, 2012, it is important that the Commission's grant
of ETC designation for T-Mobile, both for the existing ETC area granted in Order No. 32319 and
for conditional ETC designation, be consistent with the name of the entity that will submit bids
in Auction 901. The FCC has stated that "[a]n Applicant [bidder] must be the entity designated
by a State or the [FCC] as an ETC in that geographic area." DA 12-641, AU Docket No. 12-25
(issued May 2, 2012), ¶93.

For the foregoing reasons, T-Mobile requests that the Commission recognize and
acknowledge in its Order that, effective June 25, 2012, T-Mobile West Corporation will become
T-Mobile West LCC and that T-Mobile West LLC will be the designated ETC in Idaho, both for
the existing ETC designation granted in Order No. 32319 and for the conditional ETC
designation for areas outside its existing ETC area.
