On November 21,2018,Teliax,Inc ("Teliax")filed an Application with the
Commission,pursuant to Procedural Order No.32277 and IDAPA 31.01.01.114,for registration
as a retail and wholesale provider of telecommunications service in Idaho.Application at 2.
NOTICE OF APPLICATION
YOU ARE HEREBY NOTIFIED that Teliax is a corporation incorporated in the State
of Colorado and lists its principal place of business as Denver,Colorado.Id In March 2018,
Idaho's Secretary of State certified that Teliax is authorized to transact business in Idaho as of
March 8,2018.Id.;Exhibit A.Teliax lists Corporation Service Company,12550 W.Explorer
Drive,Suite 100,Boise,Idaho 83713,as its registered agent in Idaho.Id.at 2.
YOU ARE FURTHER NOTIFIED that Teliax is seeking authority to provide retail and
wholesale local telecommunications services within the State of Idaho.Id.at 1.Teliax states that
it intends to begin negotiating an interconnection agreement with Qwest Corporation dba
CenturyLinkQC upon approval of its Application.Id.at 2.
YOU ARE FURTHER NOTIFIED that Teliax states that it will comply with all
applicable Idaho laws and Commission Rules applicable to competitive wholesale providers of
local telecommunication services and,if it obtains numbering resources in Idaho,will be subject
to all applicable numbering conservation measures.Id at 3.

YOU ARE FURTHER NOTIFIED that the Commission has determinedthat the public
interest may not require a formal hearing in this matter and will proceed under Modified Procedure
pursuant to Rules 201 through 204 of the Idaho Public Utilities Commission's Rules of Procedure,

IDAPA 31.01.01.201 through 204.The Commission notes that Modified Procedure and written
comments have proven to be an effective means for obtaining public input and participation.
YOU ARE FURTHER NOTIFIED that any person desiring to state a position on this
Application may file a written comment in support or opposition with the Commission within
twenty-one (21)days from the service date of this Notice.The comment must contain a
statement of reasons supporting the comment.Persons desiring a hearing must specifically request
a hearing in their written comments.Written comments and reply comments concerning this
Application shall be mailed to the Commission and the Applicant at the addresses reflected below:

YOU ARE FURTHER NOTIFIED that if no written comments or protests are received
within the time limit set,the Commission will consider this matter on its merits and enter its Order
without a formal hearing.If written comments are received within the time limit set,the
Commission will consider them and,in its discretion,may set the same for formal hearing.
YOU ARE FURTHER NOTIFIED that the Company may file reply comments,if
necessary,within seven (7)days of the comment deadline.
YOU ARE FURTHER NOTIFIED that the Application and supporting workpapers
and exhibits have been filed with the Commission and are available for public inspection during
regular business hours at the Commission's office.The Application is also available on the
Commission's web site at www.puc.idaho.gov.Click on the "File Room"tab at the top of the

YOU ARE FURTHER NOTIFIED that all proceedings in this case will be held
pursuant to the Commission's jurisdiction under Title 62 of the Idaho Code and specifically Idaho
Code §§62-604 and 62-622.The Commission may enter any final Order consistent with its
authorityunder Title 62 and Commission Rule 114,IDAPA 31.01.01.114.
YOU ARE FURTHER NOTIFIED that all proceedings in this matter will be conducted
pursuant to the Commission's Rules of Procedure,IDAPA 31.01.01.000 et seq.
ORDER
IT IS HEREBY ORDERED that Teliax's Application be processed by Modified
Procedure,IDAPA 31.01.01.201-204.Persons interested in submitting written comments may do
so no later than twenty-one (21)days from the service date of this Order.Teliax may file reply
comments,if any,within seven (7)days of the comment deadline.
