COMES NOW Time Warer Cable Information Services (Idaho), LLC, d//a Time
Warner Cable ("Applicant"), by and through its attorneys, and hereby submits this application
for registration/certification as a wholesale telecommunications services provider within the
State of Idaho. Applicant respectfully requests that the Idaho Public Utilities Commission
("Commission") grant this Registration/Certification pursuant to Commission Order No. 32277,
in Case No. GNR-T-l 1-01, Commission Rule of Procedure 114 and Title 62 of the Idaho Code.

A. Applicant is a competitive telecommunications company that intends to
offer intrastate telecommunications services for commercial and wholesale customers
statewide. Such services may include point to point, private line, access and transport
services.
B. Applicant is a Delaware limited liability company.

3. Intereonnection Agreement. Applicant has an interconnection agreement with
Frontier Communications Northwest, Inc.
4. Complianee with Commission Rules. Applicant has reviewed all of the
Commission rules and agrees to comply with said rules, as applicable, unless otherwise ordered.
5. Release of Information. Applicant hereby permits the release of its name,
address and general business telephone number in response to a request for the same.
6. Conservation of Telephone Numbers. Applicant acknowledges that non-paging
telecommunications cariers with telephone numbering resources in Idaho shall be subject to
numbering conservation measures, including mandatory one thousand (1,000) block pooling.
