Commission approved Tel West's Certificate by Order Nos. 28909 and 29221 issued December
, 2001 and April 10, 2003 respectively, and authorized Tel West to provide
telecommunications services in the State of Idaho. This Certificate is predicated upon and is
issued pursuant to the findings of the Commission in Order Nos. 28909 and 29221.
