Dear Jan Noriyuki:
On June 14, 2022 Telrite filed an Application for Designation as an Eligible
Telecommunications Carrier (the "Application") throughout the State of ldaho. On July 1,2022 Telrite
filed a Supplement to the Application to meet certain requirements set forth in Order No. 35126
regarding the provision of service in Tribal areas.
This Second Supplement to the Application notifies the ldaho Public Utilities Commission that
Telrite provided a copy of its Application and Supplement to the below listed five federally recognized
tribes in the State of ldaho in the manner and method indicated. Copies of the delivery confirmations
are attached in listing order.
