On June 14, 2022 Telrite filed an Application for Designation as an Eligible
Telecommunications Carrier (the "Application") throughout the State of ldaho. Since then, your staff
has clarified that certain requirements set forth in Order No. 35126 must be complied with regarding
the provision of service in Tribal areas.
Please accept this filing as a Supplement to that Application.
Attached as Exhibit C-2 please find a proposed Tribal Lifeline Service Package for ldaho.
Additionally, concurrent with this filing, Telrite is providing a copy of its June 14, 2022 Application
together with a copy of this Supplement to each of the five federally recognized tribes in the State of
ldaho (the Shoshone-Bannock, the Shoshone-Paiute, the Coeur d'Alene, the Kootenai, and the Nez
Perce).
