APPLICATION
SQF, LLC, ("SQF" or "Applicant"), by its undersigned counsel, and pursuant to OrderNo.
32277 issued in CaseNo. GNR-T-11-01, IDAPA 31.01.01.114, and Title62 of the ldaho Code,
hereby applies to the Idaho Public Utilities Commission ("Commission") for a
Registration/Certification as a wholesale provider of telecommunications services in the State of
Idaho.
In support of its Application, SQF states as follows:
I. Name, Address and Form of Business (IDAPA 31.01.01.114.01)
Applicant's legal name is SQF, LLC. Applicant does not maintain a place of business in
Idaho. SQF's management team will assume responsibility for its business in Idaho and for its
Idaho operations from its headquarters in Portland, Maine.

SQF is a limited liability company organized under the laws of the State of Maine. Copies
of SQF's Articles of Organization and Certificate of Authority to transact business as a foreign
LLC in Idaho are attached hereto as Exhibits 7 and2, respectively.

SQF is a wholly-owned subsidiary of Tilson Technology Management, Inc. ("Tilson"), a
Maine corporation, which has a 100Yo membership interest in SQF. Tilson does not provide
telecommunications services except through SQF. SQF has no subsidiaries.

III. Interconnection Agreements (IDAPA 31.01.01.114.06)
SQF primarily installs its own facilities in or on existing infrastructure such as utility poles
in the public rights of way. Therefore, it typically does not require unbundled network elements
(UNEs) purchased through an interconnection agreement ("lCA") with the incumbent local
exchange carrier ("ILEC"). Should SQF determine it is not feasible to install its own facilities and
that an ILEC has suitable facilities, SQF would open negotiations for an ICA as necessary after
being granted authority by the Commission. If SQF reaches agreement on an ICA through
negotiation or arbitration, copies of the ICA will be filed with the Commission for its approval.
IV. Compliance with Commission Rules (IDAPA 31.01.01.114.07)
Attached hereto is a sworn verification executed by SQF stating that SQF agrees to comply
with all Idaho laws and Commission rules and regulations.
V. Conservation of Telephone Numbers
At this time, SQF does not hold and does not anticipate obtaining numbering resources for
ldaho. If SQF does obtain numbering resources for Idaho, however, it acknowledges that it shall
be subject to numbering conservation measures including mandatory one thousand (1,000) block
pooling. Attached hereto is a sworn verification acknowledging SQF's requirement to manage
numbering resources. To the extent SQF has telephone number resources in Idaho, SQF will
evaluate its numbering resources and donate to the numbering resource pool unused one thousand
(l ,000) number blocks and one thousand (1,000) number blocks that have fewer than ten percent
(10%) of telephone numbers assigned. SQF will file the necessary utilization reports with the
number administrator and semi-annually report their number resource utilization reports with the

number administrator and semi-annually report their number resource utilization/forecast (NRUF)
data at the one thousand (1,000) block level for each rate center within their service territory.
VI. Conclusion
SQF respectfully requests that the Commission issue SQF, LLC a
Registration/Certification as a wholesale provider of telecommunications service in the State of
Idaho.
