On May 11 , 2004, the Commission issued Order No. 29495 granting Southwestern
Bell Communications Services, Inc. dba SBC Long Distance (SBC) a Certificate of Public
Convenience and Necessity to provide resold and facilities-based local exchange and exchange
access services within the State of Idaho. On May 17, 2004, Certificate No. 426 was issued to
SBC.
On May 4, 2005, SBC notified the Commission that it had changed its corporate
form from a corporation into a limited liability company, and changed its name to SBC Long
Distance, LLC dba SBC Long Distance. There were no changes to management, operations or
the costs and types of services offered to the public as a result of the restructure. SBC submitted
a revised tariff reflecting the name change, and requested the Commission to change the name on
the Company Certificate. The Company provided a copy of its amended Certificate of
Authority issued by the Idaho Secretary of State, as well as a new performance bond (No.
6332367) reflecting the name change. SBC asked that its previous performance bond No.
6259870 be returned. At the May 17 2005 decision meeting, Staff recommended and the
Commission approved the request for the change in corporate form.
Based on our review of the Application, we find that it is reasonable to amend
Certificate of Public Convenience and Necessity No. 426 to reflect the change from
Southwestern Bell Communications Services, Inc. dba SBC Long Distance to SBC Long
Distance, LLC dba SBC Long Distance. The prior bond shall be returned.

ORDER
IT IS HEREBY ORDERED that Certificate of Public Convenience and Necessity
No. 426 be amended to reflect the name change from Southwestern Bell Communications
Services, Inc. dba SBC Long Distance to SBC Long Distance, LLC dba SBC Long Distance.
IT IS FURTHER ORDERED that performance bond No. 6259870 be returned to the
Company so it may be cancelled.
THIS IS A FINAL ORDER. Any person interested in this Order may petition for
reconsideration within twenty-one (21) days of the service date of this Order with regard to any
matter decided in this Order in Case No. SBC-04-1. Within seven (7) days after any person
has petitioned for reconsideration, any other person may cross-petition for reconsideration. See
Idaho Code ~~ 61-626 and 62-619.
