IT IS HEREBY CERTIFIED that this Certificate of Public Convenience and Necessity, Certificate No. 330, is issued to Sawtooth Telephone, Inc., a subsidiary of Midvale Telephone Exchange, Inc., to authorize Sawtooth to provide Title 61 regulated telecommunication services, including basic exchange services, within parts of the state of Idaho. The Commission approved Sawtooths Certificate by Order No. 27166 issued October 20, 1997, in Case Nos. USW-T-97-17, COL-T-97-1, COU-T-97-1, DCL-T-97-1, FMT-T-97-1, FRE-T-97-1, PRJ-T-97-1, SAW-T-97-1, WTL-T-97-1. Order No. 27166 limits the areas in which Sawtooth is authorized to provide Title 61 services and includes the following territory:
