AND DIRECT COMMUNICATIONS LAKESIDE COMMUNICATIONS, INC. FOR AN ORDER AUTHORIZING THE MERGER OF THE APPLICANT COMPANIES. ) ) ) ) ) ) CASE NO. ROK-T-98-1 FOURTH AMENDED CERTIFICATE NO. 200 IT IS HEREBY CERTIFIED that this Fourth Amendment to the Certificate of Public Convenience and Necessity, Certificate No. 200, is issued to Direct Communications Rockland, Inc. (formerly known as Rockland Telephone Company, Inc.). It is further certified that the public convenience and necessity requires, and will require, Direct Communications Rockland, Inc., its assigns and successors, to provide Title 61 and Title 62 telecommunications services, including basic local exchange services, to the inhabitants of the areas set forth and described in Certificate No. 200, as amended in 1978, 1979 and 1981, and to the inhabitants of the areas set forth and described in Certificate No. 332, issued to Direct Communications Lakeside, Inc. (formerly Lakeside Communications, Inc., and formerly a subsidiary of Direct Communications Rockland, Inc.). Certificate No. 332 and all amendments to Certificate No. 200 are made a part of this Fourth Amended Certificate by reference. The Commission approved this Fourth Amendment to Direct Communications Rockland, Inc.’s Certificate No. 200 by Order No. 27866 dated January 14, 1999, in Case No. ROK-T-98-1. In Order No. 27866, the Commission further approved the merger of Direct Communications Rockland, Inc. with its subsidiary, Direct Communications Lakeside, Inc. This Amended Certificate is issued pursuant to the findings and orders of this Commission (Order No. 2075 dated April 13, 1951, Order No. 14287 dated November 22, 1978, Order No. 14513 dated March 30, 1979, Order No. 16216 dated February 1981, Order No. 27166 dated October 20, 1997 and Order No. 27866 dated January 14, 1999).
