and Necessity, Certificate No. 336 (Second Amended), is issued to 1-800-Reconex, Inc. aka
USTel. This Certificate authorizes 1-800-Reconex aka USTel to provide telecommunications
services including basic local exchange services within the State of Idaho. The Commission
approved 1-800-Reconex aka USTel's amended Certificate by Order No. 29532 issued June 29
2004, and authorized 1-800-Reconex aka USTel to provide telecommunications services in the
State of Idaho. This Certificate is predicated upon and is issued pursuant to the findings of the
Commission in Order No. 29532.
