On August 5,2011,Qwest Corporation dba CenturyLink QC (Qwest)filed a request
for a waiver of number assignment requirements relating to an application the Company
previously filed with NeuStar,the North American Number Plan Administrator (NANPA).
Qwest received a request for additional numbers from a customer served by the Nampa,Idaho,
rate center.Qwest reviewed the customer’s request and determined it did not have the
appropriate numbering resources available within the Nampa rate center.On August 2,2011,
Qwest submitted a 1,000-block application form to NeuStar to satisfy the request of the customer
for additional numbers to meet the customer’s business needs.NeuStar denied the request due to
the months to exhaust and/or usage threshold calculations for the Nampa rate center.Qwest’s
Application filed with the Commission includes the request filed with NeuStar as a confidential
document.
Qwest’s Application asserts that the denial by NeuStar triggers the waiver process as
outlined in Federal Communications Commission Numbering Optimization Orders.The FCC
permits a waiver of the kind requested by Qwest when there is a demonstrated need,such as a
specific customer request for numbers.FCC Orders delegate authority to hear requests for
waivers to state commissions and recommends that commissions act expeditiously.
Staff reviewed Qwest’s waiver Application and the available number resources in the
Nampa rate center,and contacted Qwest to discuss the customer’s specific request.After
discussions with Qwest and the customer,it was determined that the customer can use the 208-
505-2 block (1,000 telephone numbers),avoiding the need to open a complete code (10,000
telephone numbers).This decision will help preserve 10,000 numbers in the Idaho 208 area code
inventory.

Staff reviewed the request for waiver of the ‘months to exhaust”threshold
requirements and notes that the Qwest customer is a single entity that needs these numbers as a
result of expansion of its business.These numbers will be held solely by this requesting
customer,enabling it to expand its business and meet its specific customer needs in the Nampa
community.Staff recommended the Commission approve Qwesfs requested waiver and that
1 ,000 consecutive numbers,specifically NPA NXX X (208-505-2),be released to Qwest for its
Nampa customer.
After reviewing Qwest’s Application and Staffs recommendation,the Commission
finds it appropriate to approve the waiver request filed by Qwest.The requested number
assignment is necessary for Qwest to meet the needs of its customer,and granting the request
will facilitate the business expansion of the customer.Accordingly,the Commission finds it
reasonable to direct that NeuStar release the NPA NXX X (208-505-2)block to Qwest to meet
the business expansion needs of its customer in Nampa,Idaho.
ORDER
IT IS HEREBY ORDERED that Qwest’s request for a waiver of the FCC’s
Numbering Optimization Requirement is approved.
IT IS FURTHER ORDERED that NeuStar release the NPA NXX X (208-505-2)
number block in the Nampa,Idaho wire center to Qwest to satisfy the request of a Qwest
customer.
THIS IS A FINAL ORDER.Any person interested in this Order may petition for
reconsideration within twenty-one (21)days of the service date of this Order.Within seven (7)
days after any person has petitioned for reconsideration,any other person may cross-petition for
reconsideration.See Idaho Code §61-626.
