QWEST CORPORATION'S REQUEST
FOR WAIVER
Qwest Corporation ("Qwest"), by and through its attorneys of record, and
pursuant to IDAPA 31.01.01.256, files the following Request for Waiver. Qwest
petitions the Commission for review of the denial of Qwest's application for additional
numbering resources in area code 208 by the administrator of the North American
Numbering Plan, Neustar.
This waiver filing is made in conformity with the Federal Communications
Commission s ("FCC") procedures for challenging determinations ofNeustar, outlined in

released in CC Docket No. 99-200 and FCC 01-362.
In support of this petition Qwest states:
Qwest is a telephone corporation regulated by the Idaho Public Utilities
Commission ("Commission ) providing local exchange telecommunications services in
the Idaho Falls Rate Center.
The goal of the FCC's Number Optimization Orders is to implement
uniform standards governing requests for telephone numbering resources in order to
increase efficiency in the use of existing telephone numbers and to avoid further
exhaustion of area codes under the North American Numbering Plan ("NANP"
Among other things, the Number Optimization Orders adopted revised
standards for assessing a carrier s need for numbering resources by requiring rate-center
based utilization rates to be reported to Neustar. The FCC further required that before
applicants could qualify for access to new numbering resources, applicants had to
demonstrate that the existing numbering inventory within the applicant's rate center will
be exhausted within six months of the application and is at 75% or more utilization.

On February 14, 2005 a Qwest Idaho Falls customer requested 3000
additional numbers in the IDFLIDMADS 1 switch in the Idaho Falls rate center consisting
of three blocks excluding the 0 1 and 9 blocks. (See attached letter from customer
labeled Confidential Attachment A). Qwest examined the switches in the Idaho Falls rate
center and confirmed that it did not have numbers meeting the customer s criteria.
On or about March 8, 2005, Qwest submitted an automated Part 1A and
MTE (Months to Exhaust) Worksheet to Neustar via its online system (PAS) to obtain
numbering resources necessary to meet the demands of its customer in Idaho Falls, Idaho.
The request was denied because Qwest did not meet the months-to-exhaust and
utilization requirements. (See attached request from Qwest to the Pooling Administrator
labeled Attachment B and the denial labeled Attachment C.)
Qwest's inability to provide its customer with the numbering resources it
requires for its Direct in Dial ("DID") service prevents Qwest from providing the quality
of service this customer desires and expects.
Qwest's requested numbering resources would not materially impact the
exhaust date of available numbers in the 208 area code.
The FCC allows for a waiver of the rules when there is a demonstrated
need such as a specific customer request for a large block of numbers. The waiver
process is specifically addressed in the FCC Third Report and Order as the "safety valve
process.,,3 The FCC's orders delegate the authority to hear claims for waivers to the
state regulatory commissions.

WHEREFORE Qwest respectfully requests that:
The Commission approve Qwest's request for a waiver of the FCC'
utilization threshold and months to exhaust requirements and direct the FCC's Pooling
Administrator to provide the numbering resources requested by its customer as described
in paragraph 4 above.
The Commission take up this matter at its next available decision meeting,
if possible, so that the customer s needs can be met on an expeditious basis.
