Qwest Corporation ("Qwest"), by and through its undersigned attorneys, and pursuant to
Order No. 29408 files the attached Third Revised Fourth Amended Exhibit A, which reflects the
prices for unbundled network elements (UNEs) ordered by the Commission in Case No. QWE-
01-11. Qwest understands that because these prices have been previously approved by the
Commission and because those prices that were not subject to the Commission s order are
unchanged, this Third Revised Fourth Amended Exhibit A is effective immediately.
The attached Third Revised Fourth Amended Exhibit A integrates the Commission-
ordered prices into the existing price schedule for all UNEs. In some cases the section numbers
reflected in the Attachment to Order No. 29408 have been rearranged to better align the price list

provisions with the corresponding sections of Qwest' s Statement of Generally Available Terms
(SGAT). For example, the price for "Private Line to Unbundled Loop Conversions" listed in the
Attachment to the Order No. 29408 as section 9.2.6 has been moved to section 9.2.8. Similarly,
in addition to several section number changes, the prices for "Power Reduction" elements listed
as section 8.1.4 have been moved to section 8., and section 8.14 ("Collocation Cable
Augment QFP") has been moved to section 8.1 to be consistent with the SGAT.
Section 9.
, "
Local Switching-Market Based Rates" has been moved and re-numbered
as section 9.11.8. As a result of that change, the sections following section 9.12 have been re-
numbered, thereby causing "Customized Routing," which had been listed as 9., to become
section 9.12 and so on. These numbering changes do not result in change to the price elements
or price levels.
Qwest respectfully requests that the Commission accept the attached Third Revised
Fourth Amended Exhibit A as the current Exhibit A to the Idaho SGAT.

The preliminary engineering and planning costs are included in the caged and cageless space construction charges. These engineering and planning
charges are also included in the caged and cageless quote preparation fees. Upon completion of the collocation construction, the quote preparation fee
(OPF) will be credited to the final space construction charge for the collocation job. The credit will apply to whichever OPF is applied. This exhibit currently
lists multiple OPFs based on what has been proposed on the cost docket and what was approved in the AT&T interconnection agreement. CLEC may
choose either OPF at this time.
Owest will not charge for this element until the Commission has an opportunity to review and approve a rate in a cost proceeding.
All technically feasible Vertical Switch Features are available with compatible unbundled switch ports. No monthly recurring charge applies for Basic Vertical Switch
Features. Only basic Vertical Switch Features with nonrecurring charges are listed. Nonrecurring charges are applicable whenever a feature is added. whether on new
installation, conversion, or change order activity. Vertical Switch Features not listed are included in the cost of the switch port and have a rate of zero for both recurring
and nonrecurring charges.
The Premium Port monthly recurring charge applies in addition to the Analog or Digital Line Side Port recurring charges when Premium Features are ordered with analog
or digital ports.
