On July 29, 2000, Qwest Corporation filed an Application to amend Certificate of
Public Convenience and Necessity No. 255. More specifically, Qwest requests that the
Commission amend this Certificate to reflect that U S WEST Communications has changed its
name to Qwest Corporation. The Companies formally completed their merger this month.
On July 11 , 2000, the Companies filed an amended Certificate of Authority with the
Idaho Secretary of State formally changing US WEST Communications' corporate name to
Qwest Corporation. Having formally changed its name with the Secretary of State, Qwest now
seeks to change the corporate name on Certificate No. 255.
The Commission Staff recommended that the Commission process this Application
without resorting to public hearing. Staff also recommended that the Commission direct Qwest
to submit new title pages to its Title 62 price lists and Title 61 tariffs for telecommunication
services offered within Idaho. Rather than republishing all the tariff/price list catalogs with new
pages reflecting the name change, Staff believed that it is sufficient at this time to merely change
the title pages to reflect the name change.
Based on our review of the Application, we find that it is reasonable to amend the
Certificate of Public Convenience and Necessity No. 255 to reflect the change in name from
US WEST Communications to Qwest Corporation. The Commission also finds it is reasonable
to perform this administrative function without initiating a public hearing. Finally, the
Commission adopts Staff s recommendation and directs Qwest to submit new title pages to its
Title 62 price lists and Title 61 tariff service catalogs.

IT IS HEREBY ORDERED that Qwest Corporation s Application to amend the
corporate holder of Certificate of Public Convenience and Necessity No. 255 is granted.
IT IS FURTHER ORDERED that Qwest submit new title pages to its Title 62 price
lists and Title 61 tariffs to reflect the name change.
THIS IS A FINAL ORDER. Any person interested in this Order (or in issues finally
decided by this Order) or in interlocutory Orders previously issued in this Case No. QWE-OO-
may petition for reconsideration within twenty-one (21) days of the service date of this Order
with regard to any matter decided in this Order or in interlocutory Orders previously issued in
this Case No. QWE-OO-l. Within seven (7) days after any person has petitioned for
reconsideration, any other person may cross-petition for reconsideration. See Idaho Code 9961-
626 and 62-619.
