On July 13,2011,Qwest Corporation notified the Commission of a change in its
operating entity that affects its Certificate of Public Convenience and Necessity No.255.On
April 1,2011,Qwest Communications International,inc.,and its subsidiaries including Qwest
Corporation;Qwest Communications Company,LLC;and Qwest LD Corporation,completed a
merger with CenturyLink,Inc.Now that the Qwest and CenturyLink merger is complete,Staff
recommended the Commission change the name on Certificate No.255 to reflect the proper
name of the entity conducting business under authority of the Certificate.The Company states
that new title pages reflecting the new name will be filed with the Commission.
Based on our review of the information provided to the Commission by Qwest,the
Commission finds it reasonable to issue an amended Certificate of Public Convenience and
Necessity to reflect the change in name on Certificate No.255 from Qwest Corporation to Qwest
Corporation dba CenturyLink QC.
ORDER
IT IS HEREBY ORDERED that Certificate of Public Convenience and Necessity No.
255,is amended to change the name from Qwest Corporation to Qwest Corporation dba
CenturyLink QC.Qwest will be issued an amended Certificate reflecting this name change.
THIS IS A FThAL ORDER.Any person interested in this Order may petition for
reconsideration within twenty-one (21)days of the service date of this Order.Within seven (7)
days after any person has petitioned for reconsideration,any other person may cross-petition for
reconsideration.See Idaho Code §61-626.
