Dear Ms. Jewell:
On Apnl 1, 2011, Qwest Communcatons Interational Inc. and its subsidiares including
Qwest Corpration, Qwest Communcations Company, LLC and Qwest LD Corporation
("the Operatg Entities") completed thei merger with CentuLin, Inc. As a result of
the merger, the Operatig Entities will operate with a CentuLin doing business as
("db a") nae effective August 8, 2011. The entity names are as follows:
Qwest Corporation d//a CentuLin QC
Qwest Communcations Company, LLC d//a CentuLin QCC
Qwest LD Corp. d//a CentuLin LD
The legal entity names wil remai unchanged. Revised title pages for the Operatig
Entities' varous Title 62 Tarffs/Cataogs and Pnce Lists will be filed with the
Commssion to reflect the dba change. Additional Qwest entities tht may operte in
Idaho, but not under the junsdicton of the Commssion, wil also opeate with a dba
name of CentuLin.

Commssion's recrds to reflect the above-identified dba names as necessar and feel
