On July 13,2011,Qwest Corporation notified the Commission of a change in its operating
entity that affects its Certificate of Public Convenience and Necessity No.255.On April 1.2011,Qwest
Communications International,Inc.,and its subsidiaries including Qwest Corporation;Qwest
Communications Company,LLC;and Qwest LD Corporation,completed a merger with CenturyLink,Inc.
Now that the Qwest and CenturyLink merger is complete,Qwest requested that the Commission amend
this Certificate to reflect that Qwest Corporation has changed its name to Qwest Corporation dba
CenturyLink QC.
IT IS HEREBY CERTIFIED that this Amended Certificate of Public Convenience and
Necessity is issued to Qwest Corporation dba CenturyLink QC.Certificate No.255,as previously
amended,to authorize Qwest to provide telecommunications services,including basic exchange services,
within parts of the State of Idaho.The Commission approved this name change to Certificate No.255 by
Order No.32313 issued in Case No.QWE-T-00-0 I.
