AMENDMENT NO. 26
IT IS HEREBY CERTIFIED that the present and future convenience and necessity
requires, and will require, U S WEST Communications, Inc., a Colorado corporation, its successors
and assigns, to own, hold, construct or otherwise acquire, maintain and operate facilities for
telephone service to the inhabitants of the area set forth and described in Certificate No. 255 and the
amendments to such Certificate made a part of the Certificate by reference. This amendment is
issued to U S WEST=s Certificate No. 255, as previously amended, to authorize U S WEST to
provide Title 61 regulated telecommunications services, including basic exchange services, within
parts of the state of Idaho. The Commission approved this amendment to U S WEST=s Certificate
No. 255 by Order No. 27166 issued October 20, 1997, in Case Nos. USW-T-97-17, COL-T-97-1,
COU-T-97-1, DCL-T-97-1, FMT-T-97-1, FRE-T-97-1, PRJ-T-97-1, SAW-T-97-1, WTL-T-97-1.
This amendment reflects the sale of certain exchanges and the deletion of territory from
U S WEST=s authorized service area, as described in Appendix A, attached and made part of the
Certificate by reference.
This Certificate is predicated upon and is issued pursuant to the findings of the
Commission in Order Nos. 6960, 7430, 7545, 8335, 8490, 10064, 10526, 10878, 11742, 12271,
12421, 13118, 14287, 14513, 15764, 17480, 17764, 18321, 20763, 23629, 24451, 25573, 25574,
26579 and Order No. 27166, issued October 20, 1997.
