THIS IS AN IMPORTANT NOTICE REGARDING CHANGES TO YOUR CENTURYLINK VOICE SERVICES We’re sending this notification to let you know that we are making changes that affect your CenturyLink voice services effective May 15, 2020. If you do not agree to the changes to tariffs, catalogs, schedules, and other terms and conditions, then you must take the steps discussed below by May 14, 2020. Based on applicable tariffs, catalogs, schedules and other terms and conditions, we want to remind you that you’re billed in advance for recurring monthly charges associated with your voice services, including charges for features including but not limited to caller ID and voicemail (“Voice Services”), and the applicable taxes and fees for these items. What will change? As of May 15, 2020, or as soon after that date as authorized by state regulatory commissions, your Voice Services and applicable taxes and fees will not be prorated if you cancel these services on any day other than the last day of your billing cycle. These services will continue to be available to you until the end of your billing cycle and you will not receive a refund (pro-rated credits) for any days remaining in your billing cycle after you cancel Voice Services. If you do not agree to be bound by the updated tariffs, catalogs, and rate schedules, please call CenturyLink at 1-866-554-6777 by May 14, 2020 to discuss options, which may include cancellation of your Voice Services. If you instead continue to use your Voice Services, that will constitute your acceptance of the updated tariffs, catalogs, and rate schedules. Billing and Payment For Service (Cont’d) l. The applicant or customer may be required to make a deposit to be held as a guarantee for the payment of charges for services furnished. When service is terminated, the amount of the deposit, with interests will be applied to any indebtedness to the Company. A deposit will be refunded or credited to the customer’s account after 12 months if the customer has not been delinquent in payment. The Company will pay interest on deposits pursuant to the rules and regulations of the State of Idaho. When billing is provided by a local exchange company on behalf of the Company, the local exchange company’s deposit policy applies. m. In the event that a customer's account becomes delinquent, the Company may require the prepayment of additional months of actual or estimated recurring and/or nonrecurring charges for the services ordered hereunder as a condition of the continued provision of all such services. The Company also reserves the right to eliminate or otherwise modify any discounts applicable to any service being supplied to the customer under such circumstances. n. The following charge applies where billing capabilities exist. In the event a customer requests a reprint of a monthly bill that is greater than six months old, a duplicate bill charge may apply. When billing is provided by a local exchange company on behalf of the Company, the local exchange company’s duplicate bill policy applies. • Residence, per account - Reprint on paper, per bill $5.00 • Business, per account - Reprint on paper, per bill 5.00 In the event a customer requests a reprint of a monthly bill that is greater than six months old, a duplicate bill charge may apply. When billing is provided by a local exchange company on behalf of the Company, the local exchange company’s duplicate bill policy applies. o. Where billing capability exists CenturyLink’s residential voice services and applicable taxes and fees will not be prorated if the customer cancels these services on any day other than the last day of the customer’s billing cycle. These services will continue to be available to the customer until the end of their billing cycle and the customer will not receive a refund (pro-rated credits) for any days remaining in your billing cycle after they cancel their CenturyLink voice services. Rules and Regulations (Cont’d) 9. Change In Service Arrangement p. The Company may adjust its rates and charges or impose additional rates and charges on its Customers in order to recover amounts it is required by governmental or quasi-government authorities to collect from or pay to others in support of statutory or regulatory programs. Examples of such programs include, but are not limited to, the Universal Service Fund, the Primary Interexchange Carrier Charge, and compensation to payphone service providers for use of their payphones to access the Company’s services. When a change in service arrangement involves the use of facilities or services furnished by the Company, installation charges do not apply to the facilities or services continued in use. The Minimum Service Period for the facilities or services continued in use is determined by the date of the initial acceptance thereof. 10. Restoration of Service The use and restoration of service provided in emergency situations shall be in accordance with Part 64, Subpart D of the Federal Communications Commission's Rules and Regulations, which specifies the priority system for such activities. 11. Inspection The Company, or its authorized agents, may, upon reasonable notice, make such tests and inspections as may be necessary to determine that the premises are in compliance with the terms and conditions of this Catalog, and with installation, operational, or maintenance specifications of the Company. If requirements are not met, the Company may interrupt the service at any time, without penalty to the Company. 12. Cancellation for Cause By The Company a. The Company may, upon notice, discontinue the furnishing of services to a customer, without incurring any liability, upon a violation of any of the provisions governing the furnishing of service under this Catalog or any applicable laws, rules or regulations, or upon non-payment of any sum owed to the Company. b. The Company may, upon notice, discontinue the furnishing of services to a customer without incurring any liability, if the Company determines that such action is necessary to prevent or to protect against fraud or to otherwise protect its personnel, agents, facilities, or services.
