On April 1,2011,Qwest Communications International,Inc.,and its subsidiaries
including Qwest Corporation;Qwest Communications Company,LLC;and Qwest LD
Corporation.completed a merger with CenturyLink,Inc.On July 13,2011,Qwest
Communications Company LLC (QCC)notified the Commission of a change in its operating
entity that affects its Certificate of Public Convenience and Necessity No.402.On August 4,
2011,the Commission amended Certificate of Public Convenience and Necessity No.402 to
reflect the name change of Qwest Communications Corporation to Qwest Communications
Company.LLC dba CenturyLink QCC.Order No.32307.
On January 30,2014,CenturyLink notified the Commission of its planned internal
corporate re-organization of its competitive local exchange carrier (CLEC)and interexchange
carrier subsidiaries,which included a name change from Qwest Communications Company,
LLC dba CenturyLink QCC to CenturyLink Communications,LLC.On August 15,2014,
CenturyLink notified the Commission that the re-organization was completed and requested that
Certificate No.402 be amended to reflect the name change to CenturyLink Communications,
LLC.CenturyLink also provided an Amended Certificate of Authority issued by the Idaho
Secretary of State’s office.Staff reviewed the Company’s request and recommended approval at
the Commission’s August 18,2014 decision meeting.
Based on our review of the information provided to the Commission by CenturyLink,
the Commission finds it reasonable to issue an amended Certificate of Public Convenience and
Necessity to reflect the change in name on Certificate No.402 from Qwest Communications
Company,LLC dba CenturyLink QCC to CenturyLink Communications,LLC.

ORDER
IT IS HEREBY ORDERED that Certificate of Public Convenience and Necessity No.
402,is amended to change the name from Qwest Communications Company,LLC dba
CenturyLink QCC to CenturyLink Communications,LLC.CenturyLink Communications will
be issued an amended Certificate reflecting this name change.
THIS IS A FINAL ORDER.Any person interested in this Order may petition for
reconsideration within twenty-one (21)days of the service date of this Order.Within seven (7)
days after any person has petitioned for reconsideration,any other person may cross-petition for
reconsideration.See Idaho Code §61-626.
