YOU ARE HEREBY NOTIFIED that on April 2, 2004 Qwest Communications
Corporation (QCC) filed an Application for amendment of its Certificate of Public Convenience
and Necessity. The Commission on September 3 , 2002, approved Certificate No. 402 to QCC
granting it authority to provide basic local exchange services within Verizon Northwest Inc.' s
service area. QCC now seeks to amend its Certificate to enable it to provide local services
throughout the State of Idaho, including within the service area of Qwest Corporation. QCC
asked that its Application be processed by Modified Procedure, and requested that the comment
period be shortened to 14 days.
YOU ARE FURTHER NOTIFIED that QCC requests this amendment to provide
local exchange service and vertical features to business and residential customers throughout the
State of Idaho. QCC intends to provide local services using a combination of its own facilities
and resold services and/or unbundled network elements. The Company intends to provide
service in the service areas of Qwest Corporation and the independent telephone corporations.
YOU ARE FURTHER NOTIFIED that QCC has not yet filed an initial catalog or
price list for local exchange service in the area for which it was previously certificated. QCC'
Application states it will file its catalog and price lists prior to commencement of any local
exchange services offered pursuant to its existing Certificate or as amended.
YOU ARE FURTHER NOTIFIED that the Commission has determined that the
public interest may not require a formal hearing in this matter and will proceed under Modified
Procedure pursuant to Rules 201 through 204 of the Idaho Public Utilities Commission s Rules
of Procedure, IDAPA 31.01.01.201 through .204.

YOU ARE FURTHER NOTIFIED that any person desiring to state a position on this
Application may file a written comment in support or opposition with the Commission within
fourteen (14) days from the service date of this Notice. The comment must contain a statement
of reasons supporting the comment. Persons desiring a hearing must specifically request a
hearing in their written comments. Written comments concerning this Application shall be
mailed to the Commission and the Applicant at the addresses reflected below:

pursuant to the Commission s jurisdiction under Titles 61 and 62 of the Idaho Code and that the
Commission may enter any final Order consistent with its authority under the Idaho Code.
YOU ARE FURTHER NOTIFIED that all proceedings in this matter will be
conducted pursuant to the Commission s Rules of Procedure, IDAPA 31.01.01.000 et seq.
ORDER
IT IS HEREBY ORDERED that the Application of Qwest Communications
Corporation to amend Certificate of Public Convenience and Necessity No. 402 be processed by
Modified Procedure IDAP A 31.01.01.201-204.Persons interested in submitting written
comments in this matter should do so no later than 14 days from the service date of this Order.
