Dear Ms. Jewell:
By letter dated January 30,2014, CenturyLink advised the Commission of its planned intemal
corporate reorgatization of its CLEC and Interexchange Carrier subsidiaries and its intention to
change the name of the surviving subsidiary, Qwest Communications Company, LLC dba
Century Link QCC, to CenturyLink Communications, LLC. We are pleased to advise the
Commission that that internal reorganization is complete.
Attached is the Amended Certificate of Authority issued by the Idaho Secretary of State
approving the name change from Qwest Communications Company, LLC to CenturyLink
Communications, LLC. This change took place on April 2, 2014.
Accordingly, Centurylink respectfully requests that Certificate No. 402 issued in IPUC Docket
No. QCC-T-04-1 be amended to reflect the name CenturyLink Communications, LLC. Please
