for a NPA for the St. Luke's Health System Twin Falls Location. .

In response to the conversation on Thursday October 15th concerning the request from St. Luke's
Magic Valley Medical Center for a complete NPA (10,000 numbers) PMT is including our
business plan, documentation from the NueStar pooling administration and a request letter from
St. Luke's Health System.
PMT's business plan is straightforward:
. Apply for and receive a State Waiver
. Reapply to NueStar for an entireNXX NPA
. Activate within our switch the assigned NP A
. Tum up the NP A as 10,000 DID numbers on PMT services to St Lukes
. St. Luke's MVMC to turn up the numbers to their complex of locations
We have been assured by St. Luke's they wil fully utilize the NPA in the near future. As you
can see from their attached letter they are currently using over 2,200 numbers and expect
tremendous growth when moving into their new facility.
