AMENDMENT TO APPLICATION
Onvoy, Inc. d/b/a Onvoy Voice Services ("Onvoy" or "Applicant"), by its undersigned,
hereby amends its Application to include the following information:
I.Subsidiary Information (IDAPA 31.01.01.1 14.01(c)(vi)
Applicant has two subsidiaries that are public utilities in other states: Minnesota
Independent Equal Access Corporation and Zayo Enterprise Networks, LLC. Applicant has
another recently acquired direct subsidiary, VoIP360, Inc., that is not a public utility. An
updated corporate ownership chart that includes VoIP360, Inc. is provided as Replacement
Exhibit 4. All of these subsidiaries have the same address as Onvoy:

II.Description of Proposed Services
Onvoy is a competitive telecommunications company that intends to offer intrastate
telecommunications services for commercial and wholesale customers statewide. Such services
may include, but will not be limited to, wholesale local exchange and long distance, competitive

tandem switching, switched access and other access services, transit, intrastate PR! service,
direct inward dialing and SS7 services.
