Dear Wayne:
On November 17, 2003, Northwest Telephone, Inc. submitted an application to become a
competitive local exchange carrier and an inter-exchange carrier in Idaho. We now
understand that it is not necessary to request a certificate if we do not intend to provide
telephone services to residential or small business customers. Therefore, we hereby request
that our request for a certificate to become a CLEC be revised to a notice to become a CLEC.
We would like to request confirmation that Northwest Telephone, Inc has submitted the
proper notice and is now a CLEC authorized to act as such in the State of Idaho. We will
need this to enable NTI to proceed with entering into interconnection and collocation
agreements with the ILECs, RBOCs and other carriers in Idaho.
In our application submitted on November 17, 2003, we included price lists for services we
intend to provide in Idaho. Please review the information in our application (notice) to
determine if you need any additional information to approve Northwest Telephone, Inc. as an
inter-exchange carrier in the State ofidaho.
We look forward to receiving our approval and beginning to provide telecommunications
services in the State ofldaho.
