A. Sheet Numbering Sheet numbers appear in the upper rightcorner of the page. Sheets are numbered sequentially. When a new
sheet is added between sheets already in effect / a decimal isadded. For example / a new sheet added between sheets 14 and would be 14.
B. Sheet Revision Numbers - Revision numbers also appear in the
upper right corner of each page. These numbers are used to
determine the most current sheet version on file with the IPUC.
For example / the 4th revised Sheet 14 cancels the 3rd revised Sheet14. Consul t the Check Sheet for the sheet currently in effect.
C. Paragraph Numbering Sequence There are nine levels of
paragraph coding. Each level of coding is subservient to its nexthigher level:

SECTION 1 - TECHNICAL TERMS AND ABBREVIATIONS
Access Line - An arrangement which connects the Customer's location
to a North American Telephone Network, L.C. switching center or
point of presence.
Authorized User - A person/ firm , corporation, or any other entity
authorized by the Customer to communicate utilizing the Carrier'service.
Available Usage Balance - The amount of usage remaining in a Debit (N)
Account at any particular point in time. Each Debit Account begins wi th an initial usage amount which is depleted as services provided
by the Company are utilized by the Customer. (N)
Customer - The person/ firm , corporation or other entity which
orders / cancels / amends or uses service and is responsible for
payment of charges and compliance with the Company's tariff.
Company or Carrier North American Telephone Network,
unless otherwise clearly indicated by the context.
Commission - The Idaho Public Utilities Commission.
Debit Account - An account which consists of a pre-paid usage (N)
balance depleted on a real time basis during each Debit Service
Call.
Debit Card - A card issued by the Company which provides the
Customer with a Personal Account Code and instructions foraccessing the Carrier's network.
Debit Service Call - A service accessed via a Toll Free (i.
"1-800/1-888") number or other access code dialing sequence whereby
the Customer or Authorized User dials all of the digits necessary
to route a call. Network usage for each call is deducted from the
available usage balance on a Company-issued Debit Account.(N)
Dedicated Access - See Special Access Origination/Termination.
NATN - Used in this tariff to refer to North American Telephone
Network , L.C. unless clearly indicated by the text.

SECTION 1 - TECHNICAL TERMS AND ABBREVIATIONS, CON IT.
Personal Account Code - A pre-defined series of numbers to be (N)dialed by the Customer or Authorized User upon access to theCarrier's network which identifies the Debit Account from which
charges for service shall be debited and which validates thecaller's authorization to use the services provided.
Renewal - A method of replenishing a Debit Account's Available
Usage Balance with additional minutes of use as authorized and paid
for by the Customer.
Retailer person or other legal entity who contracts todistribute services of the Company at terms and condi tionsspecified in this tariff. (N)
Special Access Origination/Termination - Where access between the
customer and the interexchange carrier is provided on dedicatedcircuits. The cost of these dedicated circuits is billed by the
access provider directly to the end user.
Subscriber - The person / firm or corporation / or other entity which
orders / cancels / amends / or uses service / and authorizes NATN to
pre subscribe the service to NATN' s network.
Switched Access - Where access between the Customer and the Carrier
is provided on local exchange company circuits capable of accessing
the local switched network. The cost of switched Feature Group
access is billed to the Carrier.
Terminal Equipment - Devices / apparatus / and associated wiring
such as teleprinters / telephones / or data sets.
V & H Coordinates - Geographic points which define the originating
and terminating points of a call in mathematical terms so that the
airline mileage of the call may be determined. Call mileage is
used for the purposed of rating calls.

Undertaking of North American Telephone Network, LLC.
NATN'services and facilities are furnished for
communications originating at specified points within the
state of Idaho under terms of this price list.
NATN installs / operates / and maintains the communications
services provided hereinunder in accordance with the terms and
condi tions set forth under this price list. NATN may act as
the Customer's agent for ordering access connection facilities
provided by other carriers or entities, when authorized by the
Customer / to allow connection of a Customer's location to the
NATN network. The Customer shall be responsible for all
charges due for such service arrangement.
The Company'services and facilities are provided on a
monthly basis unless otherwise provided, and are available
twenty-four hours per day / seven days per week.
Limitations
presubscribed service is offered in Equal Access
areas only.
Service is offered subject to the availability ofthe necessary facilities and equipment, or bothfacilities and equipment, and subject to the
provisions of this price list.
NATN reserves the right to discontinue or limit
service when necessitated by conditions beyond itscontrol/or when the Customer is using service in
violation of provisions of this price list / or in
violation of the law.
The Company does not undertake to transmi t
messages/ but offers the use of its facilities when
available, and will not be liable for errors in
transmission or for fail ure to establish
connections.

'2.Use
All facilities provided under this price list are
directly or indirectly controlled by North American
Telephone Network / LLC. and the Customer may not
transfer or assign the use of service or facilitieswithout the express written consent of theCompany. Such transfer or assignment shall only
apply where there is no interruption of the use or
location of the service or facilities.
Prior wri tten permission from the Company isrequired before any assignment or transfer. All
regulations and conditions contained in this price
list shall apply to all such permitted assignees or
transferees / as well as all conditions of service.
Services provided under this price list may be used for any
lawful purpose for which the service is technically suited.
Liabilities of the Company
NATN'liabili ty for damages arising out
mistakes / interruptions / omissions / delays / errors /or defects in transmission which occur in the
course of furnishing service or facilities, in no
event shall exceed an amount equivalent to the
proportionate charge to the Customer for the period
during which the faults in transmission occur.
The Company shall not be liable for claim or loss
expense or damage (including indirect, special or
consequential damage) / for any interruption, delay /error / omission , or defect in any service / facility
or transmission provided under this price list / if
caused by any person or enti ty other than the
Company / by any malfunction of any service or
facility provided by any other carrier / by an actof God, fire , war / civil disturbance, or act of
government / or by any other cause beyond the
Company's direct control.

Liabilities of Company, con/to
The Company shall not be liable for / and shall befully indemnified and held harmless by Customeragainst any claim or loss, expense/ or damage
(including indirect, special or consequential
damage) for defamation, libel/slander / invasion,infringement of copyright or patent / unauthorized
use of any trademark, tradename or service mark,unfair competi tion / interference wi th
misappropriation or violation of any contract /
proprietary or creative right, or any other injury
to any person / property or entity arising out of
the material, data, information, or other content
revealed to/ transmitted, or used by the Company
under this price list; or for any act or omission
of the Customer; or for any personal injury or
death of any person caused directly or indirectlyby the installation, maintenance location,condition, operation, failure, presence / use or
removal of equipment or wiring provided by the
Company / if not directly caused by negligence of
the Company.
No agent or employee of any other carrier shall be
deemed to be an agent or employee of the Company.
The Company shall not be liable for any defacementof or damages to the premises of a Customer
resul ting from the furnishing of service which is
not the direct result of the Company s negligence.
The above tariff language does not constitute a
determination by the Commission that a limit ofliabili ty imposed by the Company should be upheld
in a court of law. Acceptance for filing by the
Commission recognizes that it is court' sresponsibility to adjudicate negligence and
consequential damage claims. It is also the
court's responsibility to determine the validity of
the exculpatory clause.

Deposits
The Company does not collect deposits from Customers.
Advance Payments
For Customers whom the Company determines an advance payment
is necessary / NATN reserves the right to collect an amount not
to exceed one ( 1 ) month's estimated charges as an advance
payment for service. This will be applied against the next
month's charges and a new advance payment may be collected for
the next month.
Taxes
All state and local taxes (i. e. / gross receipts tax, sales
tax, municipal utilities tax) are listed as separate line
items and are not included in the quoted rates.
Terminal Equipment
The Company's facilities and service may be used with or
terminated in Customer-provided terminal equipment or
Customer-provided communications systems / such as a telephone
set / PBX or key system. Such terminal equipment shall befurnished and maintained at the expense of the Customer /
except as otherwise provided. The Customer is responsible for
all costs at his or her premises, including personnel, wiring,
electrical power / and the like / incurred in the use of the
Company's service. When such terminal equipment is used, the
equipment shall comply with the generally accepted minimumprotecti ve criteria standards of the telecommunications
industry as endorsed by the Federal Communications Commission.

Installation and Termination
Service is installed upon mutual agreement between the
Customer and the Company. The service agreement does notal ter rates specified in this price list.
10 Payment for Service
The Customer is responsible for payment of all charges forservices and equipment furnished to the Customer or to an
Authorized User of the Customer by NATN. All charges due by
the Customer are payable to the Company or to the Company'
authorized billing agent. Terms of payment shall be according
to the rules and regulations of the billing agent and subject
to the rules of regulatory agencies / such as the IPUC. Any
objections to billed charges must be reported to the Companyor its billing agent. Adjustments to Customer's bills shallbe made to the extent that circumstances exist which
reasonably indicate that such changes are appropriate.
11 Cancellation by Customer
Customer may cancel service by providing 30 days written
notice to the Company.
2 . 12 Interconnection
Service furnished by NATN may be connected with the servicesor facilities of other carriers. Such service or facilities,
if used / are provided under the terms / rates and conditions ofthe other carrier. The Customer is responsible for all
charges billed by other carriers for use in connection with
NATN's service. Any special interface equipment or facilities
necessary to achieve compatibility between carriers is the
responsibili ty of the Customer.

13 Refusal or Discontinuance by Company
NATN may refuse or discontinue service under the following
conditions. Unless otherwise specified , the Customer will be
given five (5) days written notice and allowed a reasonable
time to comply with any rule or remedy any deficiency.

For non-compliance with and/or violation of any State ormunicipal law / ordinance or regulation pertaining totelephone service.
For the use of telephone service for any other property
or purpose other than that described in the application.
For failure or refusal to provide the Company with a
deposit or advance paYment to insure payment of bills in
accordance with the Company's regulations or failure to
meet the Company's credit requirements.
For neglect or refusal to provide reasonable access to
the Company for the purpose of inspection and maintenance
of equipment owned by the Company.
For non-compliance with and/or violation
Commission'regulations or the Company'rules
regulations on file with the Commission.

14 Inspection, Testing and Adj ustment
Upon reasonable notice , the facilities provided by the Carriershall be made available to the Carrier for tests and
adjustments as may be deemed necessary by the Carrier for
maintenance. No interruption allowance will be granted for
the time during which such tests and adjustments.

15 Tests, Pilots, Promotional Campaigns and Contests
The Carrier may conduct special tests / pilot programs / waivers
and promotions to demonstrate the ease of use / quality of
service and to promote the sale of its services.
16 Interruption of Service
Credit allowances for interruptions of service which are not
due to the Carrier's testing or adjusting / to the negligence
of the Customer / or to the failure of channels, equipment or
communications systems provided by the Customer / are subject
to the general liability provisions set forth in section 2.herein. It shall be the obligation of the Customer to notify
Carrier immediately of any interruption in service for whicha credit allowance is desired by Customer. Before giving such
notice / Customer shall ascertain that the trouble is not
wi thin his or her control/or is not in wiring or equipment
if any / furnished by Customer and connected to Carrier'terminal. Interruptions caused by Customer-provided orCarrier-provided automatic dialing equipment or access-code
programmed PBXs are not deemed an interruption of service as
defined herein since the Customer has the option of using the
long distance network via local exchange company access.

SECTION 3 - DESCRIPTION OF SERVICE
General
North American Telephone Network, Inc. offers long distance
telecommunications services to residential and businessCustomers. Intrastate service is provided only to Customers
of NATN' s companion interstate offerings. Descriptions and
rates contained in this tariff apply to the intrastate portion
of the Carrier's services.
Services provided by NATN include direct dialed (1+) service
and travel card service. Direct dial services are available
via Switched or Dedicated Access Origination or Termination.
When a Customer elects to use Travel Card service / calls may
be initiated from any location from which the caller can dial
the appropriate access codes.

Timing of Calls
Long distance usage charges are based on usage of
NATN's service. Usage timing begins upon receipt
of answer supervision from the called party at thedistant end. Call duration timing will commence
from that point.
Chargeable time for a call ends upon disconnection
by either party.
In calculating the total time for a particularcall/any fraction of the last increment of use / is
rounded up to the next higher increment.
Minimum call durations / usage measurements and
rounding increments are specified on a per-product
basis in Section 4 of this tariff.
No charges apply for incomplete calls. If a
Customer believes he or she has been incorrectly

Usage charges for all mileage sensitive products are based on
the airline distance between the serving wire center locations
associated with the originating and terminating points of thecall.
The distance between the originating and terminating points is
calculated by using the "V" and "H" coordinates of the serving
wire centers as listed in NECA FCC Tariff No.4 and defined by
BellCore (Bell Communications Research) / in the following

NATN One Plus Services
NATN One Plus Services are available for business
and residential Customers who; 1) subscribe their
local access lines to NATN' s network, 2) dial the
Company's access code to gain access to the NATN
network / or 3) purchase dedicated access facilities
from other service providers to connect their
premises to NATN' s network facilities.
NATN One Plus Services are listed in Section 4following. The minimum and additional billing
increments, as well as any applicable recurring and

NATN Travel Card Services
NATN Travel Card Service permits Customers to place
calls using NATN' s service when away from their
primary place of business or residence. Callers

Directory Assistance is available to Customers of
North American Telephone Network, Inc. Directory
Assistance charge applies to each call to the
Directory Assistance Bureau. Up to two requests
may be made on each call to Directory Assistance.The Directory Assistance charge applies to each
call regardless of whether the Directory Assistance
Bureau is able to furnish the requested telephone
number.
Directory Assistance / Per Call $0.
Contract Services
At the option of the Company / services may beoffered on a contract basis to meet specialized
requirements of the Customer not contemplated in
this tariff. The terms of each contract shall be
mutually agreed upon between the Customer and the
Company and may include discounts off of ratescontained herein / waiver of recurring nonrecurring charges / charges for specially
designed and constructed services not contained in
the Company s general service offerings / or other
customized features. The terms of the contract may
be based partially or completely on the term andvolume commitment, type of originating
terminating access / mixture of services or otherdistinguishing features. Service shall
available to all similarly situated Customers for
six months after the initial offering to the first
contract Customer for any given set of terms.

NATN Operator Services
NATN Operator Services are available to subscriber
locations which offer their telephones for the use of the
transient public. NATN provides the following billing
arrangements for End Users of NATN' s Operator Services:
Customer Dialed Calling/Credit Card
This is a service whereby the end user dials all ofthe digits necessary to route and bill the call
wi thout any operator assistance. Such calls may be
billed either to a telephone company issued calling
card or a commercial credit card.
Operator Station
This is a service whereby the caller places a
non-person-to-person call with the assistance of an
operator (live or automated). When placing an
operator station call/the caller is connected to a
non-specified individual at the terminating end.
Such calls may be billed to a calling card, credit
card , the called number (collect) or a valid third
party telephone number.
Person -to- Person
This is a service whereby the person originating
the call specifies to NATN' s operator a particular
person to be reached / or a particular person /
station / room number / department / or office to be
reached through a PBX attendant. Person-to-person
calls may be billed to a calling card , credit card

telephone number.

Exemptions and Special Rates
Discounts for Hearing Impaired Customers
telephone toll message which is communicated
using a telecommunications devise for the deaf
(TDD) by properly certified hearing or speech
impaired persons or properly certified business
establishments or individuals equipped with TDDs
for communicating with hearing or speech impaired
persons will receive, upon request/ credit on
charges for certain intrastate toll calls placedbetween TDDs. The credi to be gi ven on a
subsequent bill for such calls placed between TDDs
will be equal to applying the evening rate duringbusiness day hours and the night/weekend rate
during the evening rate period. Discounts do not
apply to surcharges or per call add-on charges foroperator services when the call is placed by a
method that would normally incur the surcharge.
Emergency Call Exemptions
The following calls are exempted from all charges:
Emergency calls to recognizable authorized civil
agencies including police, fire, ambulance, bomb
squad and poison control. NATN will only handle
these calls if the caller dials all of the digits
to route and bill the call. Credit will be given
for any billed charges pursuant to this exemption
on a subsequent bill after verified notification bythe billed Customer wi thin thirty (30) days ofbilling.

Late Payment Charge
NATN reserves the right to charge the maximum late payment
fee allowable by the Commission on any past due balance. In
the event that the Company incurs fees or expenses/ includingattorney'fees / collecting or attempting to collect / any
charges owed to the Company / the Company may charge the
Customer all such fees and expenses reasonably incurred.
Return Check Charge
NATN reserves the right to charge the maximum return check
charge for checks returned for insufficient funds. Any
applicable return check charges will be assessed according to
the terms and conditions of the billing entity (i. e. local
exchange company and/or commercial credit card company) and
pursuant to Idaho law and Commission regulations.

25 NATN Debit Card Service
(N)
25.General Description
Debit Card Service allows Customers to place direct
dialed calls between locations within the state of
Idaho. Customers access the NATN network by dialinga Toll Free (i. e. "800/888) number or other access
dialing sequence and entering a Personal Account
Code. The Company's system informs the Customer of
the Available Usage Balance remaining in his/her
Debit Account and prompts the Customer to place a
call by entering a destination telephone number.
Network usage for calls placed is deducted from the
Available Usage Balance in the Customer's account
on a real time basis as the call progresses.
Customers purchase a Debit Card which assigns each
Customer a Debit Account / provides each Customer
wi th a Personal Account Code and lists instructionsfor accessing and using Carrier's service. Debit
Cards are available in varying denominations.
Purchase of a Debit Card entitles the Customer touse the NATN network for a number of minutes
equivalent to the card denomination divided by theeffecti ve per minute rate. The Customer I s right to
utilize network usage wi thin a given Debit Account
expires one year after issuance and activation of
the Debit Card associated with that Debit Account
number. No minimum service period applies. Payment
for Debit Card Services and any Available Usage in
a Customer's Debit Account is nonrefundable. Any
unused balances will be applied toward renewal
value of the card account.
Debi t Card service rates are not distance or time
of day sensitive. Holiday discounts do not apply.
Network usage for Debit Card Calls is deducted fromthe Available Usage Balance in Customer I S Debit
Account in full minute increments. For debiting
purposes / the minimum call usage is one (1) minute;
one domestic minute is equal to one unit. (N)

25.Debi t Card Sponsor Program
(N)The Debi t Card Sponsor Program is offered toorganizations or commercial entities for
distribution to their members / patrons orcustomers. The marketing vehicle and expiration
period is selected by the Sponsor upon joint
agreement between the Company and the Sponsor. The
Sponsor is responsible for obtaining all necessary
permissions for the use of any trade mark / trade
name / service mark or other image on the card. The
Sponsor may distribute the Carrier I s debit card
accounts at reduced rates or free of charge to endusers. At the option of the Sponsor / these cards
may not be replenishable. The Company reserves the
right to approve or reject any image and to specify
the customer information language and use of theCarrier's trade mark, trade name / service mark or
other image on the card.
