IT IS HEREBY CERTIFIED that this Certificate of Public Convenience and Necessity is issued to Maxcess, Inc., Certificate No. 375, to authorize Maxcess to provide resold basic and facilities-based local exchange services, as a competitive local exchange carrier within the service territories of U S WEST Communications, Inc. and GTE Northwest Incorporated in the state of Idaho. The Commission approved Maxcess’ Certificate by Order No. 28413 issued June 15, 2000. This Certificate is predicated upon and is issued pursuant to the findings of the Commission in Order No. 28413.
