Public Convenience and Necessity No.366 to reflect that MVX.Com Communications,Inc.has
changed its name to QuantumShiftCommunications,Inc.
IT IS HEREBY CERTIFIED that this Amended Certificate of Public Convenience and
Necessity is issued to QuantumShift Communications,Inc.,Certificate No.366,to authorize
QuantumShift to provide competitive local exchange services within the state of Idaho.The
Commission approved this name change to Certificate No.366 by Order No.28776 issued July 10,
2001,in Case No.MVC-T-01-1.
