Certificate of Public Convenience and Necessity Number 366 issued on December 7,1999 in Case
No.GNR-T-99-16 to reflect the name change to QuantumShift Communications,Inc.
Copies of QuantumShift Communications,Inc.'s initial local exchange tariff and amended
Certificate of Authority from the Department of State reflecting the name change were filed on
May 11,2001.QuantumShift Communications,Inc.'s intrastate telecommunications tariff was
filed on May 7,2001.Please cancel any tariffs on file for MVX.COM Communications,Inc.
