Dear Ms. Hanian,
Pursuant to Idaho Code §62‐606, McLeodUSA Telecommunications Services, LLC (“McLeodUSA”) hereby
notifies the Idaho Public Utilities Commission (“Commission”) of McLeodUSA’s election to provide retail
services on a detariffed basis; McLeodUSA requests this election to become effective September 1, 2018.
This request effectively cancels the following document currently on file with the Commission:
Title 62 Price List, titled Intrastate, IntraLATA, Interexchange and Local Exchange Services Tariff No. 3
McLeodUSA provides regulated voice services to business customers only, via individually negotiated
service contracts. This request to elect deregulation will have no impact on McLeodUSA’s existing
customers terms and rates as provided for in the customer contracts. As required by the Commission,
and to ensure customers are aware of this request, McLeodUSA has notified its customers with services
in Idaho of its detariffing petition via the enclosed bill message, which will be sent to customers thirty (30)
days prior to the requested effective date above.
As stated in the customer notification, the General Terms & Conditions of Service are accessible online at
https://www.windstream.com/about/legal/Terms‐and‐Conditions.
Additionally, the retail tariff being withdrawn will be available in the form of a Service Guide, by selecting
the customers respective Windstream company, in this case McLeodUSA,
at http://www.tariffs.net/windstream/.
