Idaho Statutes - Chapter 6, Section 62-622.

Dear Secretary,

Metropolitan Telecommunications of Idaho, Inc., d/b/a “MetTel” seeks withdrawal of
the company’s local exchange tariff (“Idaho Tariff Number 4”) pursuant to the terms and
provisions set forth in the above captioned statute.

 MetTel will replace its local tariff with a Product Guide.
 The MetTel Product Guide will appear on the company’s public website
(http://www.mettel.net/tariffs.html).
 MetTel will provide written notice to all Idaho customers and
subscribers.

MetTel seeks an Effective Date of January 8, 2015 for both the activation of the Product
Guide and the withdrawal of the company’s Idaho Tariff Number 4.

Also enclosed is an extra copy of this letter. Please date stamp this extra copy and return
it in the enclosed self-addressed stamped envelope. This letter has also been submitted
electronically.
