On July 21, 2023, Metropolitan Telecommunications of Idaho, Inc. filed an Application with the
Idaho Public Utilities Commission (“Commission”) asking the Commission to amend the Certificate of
Public Convenience and Necessity (“CPCN”) for Metropolitan Telecommunications of Idaho, Inc. to
reflect that Metropolitan Telecommunications of Idaho, Inc. converted to a limited liability company
and has changed its name to Metropolitan Telecommunications of Idaho, LLC.
IT IS HEREBY CERTIFIED that this First Amended CPCN, Certificate No. 448, is issued to
Metropolitan Telecommunications of Idaho, LLC. This Certificate authorizes Metropolitan
Telecommunications of Idaho, LLC to provide resold and facilities-based local exchange services within
the State of Idaho. This Certificate is predicated upon and is issued pursuant to the findings of the
Commission in Order No. 36024.
