as a Competitive Local Exchange Carrer in the State of Idaho. Applicant currentlyCèprovides
wireless internet, RF cable television and cable modem service to over 700 residents and
businesses in southeast Idaho, through its affiliate ICS of Idaho, LLC dba Milennium
Broadband. Applicant intends to provide VoIP telecommunication serices to its current
customers and to potential customers in Idaho and surounding states.
Applicant is a facilities-based provider that intends to install and upgrade its equipment to
provide professional-grade telecommunications. Applicant wil market its product directly to its
current customers via its web site, bil stuffers and other professional communications, such as
radio, television and direct mail marketing material, to inform the public of its available services.
II. Form of Business
1. Applicant is a Wyoming limited liability company, registered with the Wyoming
Secretary of State on Januar 29, 2004; a certified copy of Applicant's Aricles of Organization
is attàched hereto as Exhibit "A."
2. Applicant is registered to do business in the State of Idaho as a foreign limited
liability company; a certificate of existence for Applicant issued by the Idaho Secretar of State
is attached hereto as Exhibit "8."

III. Telecommunication Servce
1. Applicant has the necessary equipment to provide serices immediately after
CLEC status is granted.
2. Applicant wil provide serices to residential and commercial subscribers, in the form
of broadband and dial-up Internet access and proposes to provide voice over Interet protocol
(VoIP) telecommunications services.
IV. Service Territory
1. Applicant wil compete with Qwest and others. A copy of Applicant's service
terrtory map is attached as Exhibit "c."

2. Applicant has no intention, nor anticipation, of facilities constrction at this time.
3. Applicant may compete with Vonage, Skye and other similar VoIP service
providers.
4. Applicant is a fiber and coaxial based provider and wil provide its services from
Soda Springs, Idaho. These facilities contain necessary bandwidth originating from fiber optic
cable.

VIII. Interconnection Agreements
Applicant has negotiated an interconnection agreement for its services in the State of
Idaho with Qwest through its affliate Silver Star Communications. Applicant wil initiate such
other necessary interconnection negotiations after this Application is granted.
IX. Compliance with Commission Rules
Applicant has reviewed all of the Commission rules and agrees to comply with them, or
to request for waiver of those rules believed to be inapplicable.

Applicant does not currently intend to require advance customer deposits.
