SUBJECT: MIDVALE TELEPHONE EXCHANGE'S APPLICATION CONCERNING
INVESTMENT TAX CREDIT FOR INSTALLING QUALIFIED
BROADBAND EQUIPMENT, CASE NO. MID-T-08-01
On October 29, 2008, Midvale Telephone Exchange fied an Application for
broadband investment tax credit for 2006 and 2007. The Company requested authorization of
$277,466 in broadband investments for the year ending December 31, 2006, and $634,226 in
broadband investments for the year ending December 31, 2007. After a preliminar review,
Commission Staff notified the Company that more information was required before its
Application could be processed. The Company voluntarily withdrew its Application on March 6,
2009, advising the Commission that it would "re-fie those applications when time permits."
Consequently, this case should now be closed.
