On August 18, 2003, MCImetro Access Transmission Services, Inc. requested to
amend Certificate of Public Convenience and Necessity No. 337 to reflect that it has changed its
name to MCImetro Access Transmission Services, LLc.
IT IS HEREBY CERTIFIED that this First Amended Certificate of Public
Convenience and Necessity No. 337 is issued to MCImetro Access Transmission Services, LLC
to provide local telecommunications services, including basic exchange services, within parts of
the State of Idaho. See attached map. The Commission approved the name change from
MCImetro Access Transmission Services, Inc. to MCImetro Access Transmission Services, LLC
by Order No. 29367 issued October 23, 2003. The Amended Certificate is predicated upon and
is issued pursuant to the findings of the Commission in Order No. 29367.
