Proposed Services
My Choice Communications, Inc., d/b/a MCCI ("MCCI" or "Applicant"
seeks authority to provide resold and/or facilities-based (leased) competitive
local exchange telecommunications services. Applicant seeks authority to
provide these services in Idaho exchanges currently served by Qwest. The
local exchange services Applicant proposes to offer include residential and
small business basic exchange service, including touchtone dial-tone services
and vertical features, such as voice mail , caller 10 , call waiting, and other
standard calling features. Applicant also proposes to offer business class
local loop connectivity and transmission, including 2-wire , 4-wire, 05-, 05-
, and 05-local loops necessary to connect business customers to
interexchange carrier points of presence (POPs).

based competitive local exchange telecommunications services.
Applicant seeks authority to provide these services in Idaho
exchanges currently served by Qwest. The local exchange
services Applicant proposes to offer include residential and small
business basic exchange service, including touchtone dial-tone
services and vertical features, such as voice mail , caller 10 , call

proposes to offer business class local loop connectivity and

Applicant seeks authority to provide competitive local exchange
services to customers residing in Qwest's Idaho exchanges.

of the property owned or controlled by applicant.
Initially, Applicant seeks authority to provide resold and leased
local exchange services. Applicant will lease Qwest's unbundled
network element platform.
Applicant does not anticipate installing physical plant at this time.
As the market for its services and business plans dictate
however, Applicant may eventually lease or deploy its own
switches within existing carrier hotels or other leased premises.
No physical construction of fiber optic or copper cables is
anticipated , now or in the future.

Applicant's local exchange services will be sold in competition with
Qwest, AT&T, MCI, Level 3 and other non-rural incumbent LECs
and CLECs currently providing services in Idaho exchanges served

profit and loss statements of applicant reflecting current and prior
year balances for the twelve months ended as of the date of the
balance sheet, or if not readily available, for the period since the
close of the preceding calendar year.
If a balance sheet and income statement are not available, the
applicant must submit financial data sufficient to establish that it
possesses adequate financial resources to provide the proposed
services.

VII!. Interconnection Agreements
1. Statements of whether the applicant has initiated interconnection
negotiations and, if so, when and with whom.
Applicant is presently negotiating an interconnection agreement
with Qwest.
IX.Compliance with Commission Rules
written statement that the applicant has reviewed all of the
Commission rules and agrees to comply with them or a request for
waiver of those rules believed to be inapplicable.

Escrow Account or Security Bond
If a company requires advance deposits by its customers, the
company must submit a signed copy of an escrow account with a
bonded escrow agent or a security bond. The escrow or bond shallbe sufficient to meet customer deposit refunds in case
company default.
Applicant does not require advance deposits by its customers.
At the Commission s discretion , an additional deposit may be
required to keep customers whole in case of company default.
The Commission will review the individual requirement of
establishing an escrow or security account by the Company upon
good showing by the Company for a period of two years.

The name of the Corporation shall be MY CHOICE COMMUNICATIONS, INC.
ARTICLE n
The Corporation shall have perpetual existence, and the nature of the business or
purposes to be conducted or promoted by the Corporation is to provide
communications goods and
services and to transact any and all other lawful business for which corporations may be organized
under the mCA.
ARTICLE ill
The address of the initial registered office 01 the Corporation in the State of Idaho is
201 E. Center Street, Pocatello, Idaho 83204. The name of its registered agent at such address is
ERIC L. OLSEN.
ARTICLE IV
The aggregate number of shares which this Corporation shall have the authority to
issue is One Million (1,000 000) shares, as follows:
(1) Common Stock.Of the total authorized capital stock, the Corporation
shall have the authority to issue One Million (1,000 000) shares with no par value, which shares
shall be divided into two classes designated as "Class A Voting Common Stock" and "Class B
Non-Voting Common Stock" respectively.
(a) Class A Voting Common Stock. Nine Hundred Thousand (900 000)
shares shall be designated as Class A Voting Common Stock. Each share of Class A
Voting Common Stock shall have unlimited voting rights. Other than with respect to
voting rights, Class A Voting Common Stock and Class B Non-Voting Common Stock
shall have identical rights, including but not limited to the right to receive dividends when
and as declared by the Board of Directors of the Corporation in accordance with
applicable law.

(b) Class B Non-Voting Common Stock. One Hundred Thousand (100 000)
shares shall be designated as Class B Non-Voting Common Stock. The shares of Class B
Non-Voting Common Stock shall have no voting rights. No separate vote of the holders
of Class B Non-Voting Common Stock as a class shall be required for any purposes
except as may be required by law. Other than with respect to voting rights, Class A Voting
Common Stock and Class B Non-Voting Common Stock shall have identical rights
including but not limited to the right to receive dividends when and as declared by the
Board of Directors of the Corporation in accordance with applicable law.
(2) The holders of Class A Voting Common Stock will be entitled to one vote on
each matter submitted to a vote at a meeting of stockholders for each share of Class A Voting
Common Stock held of record by such holder as of the record date for such meeting-

Section 5.All corporate powers shall be exercised by or under the authority of,
and the business and affairs of the Corporation managed under the direction of, its Board of Directors,
subject to any limitation in a shareholder agreement authorized under Section 30-732 of the mCA

No shareholder of this Corporation shall have any preemptive rights with respect to (i)
any shares of stock of the Corporation. whether now or hereafter authorized, (ii) any warrants, rights
or options to purchase any such shares, or (iii) any obligations convertible into any such shares or into
warrants, rights or options to purchase any such shares.

Senior Systems Engineer: Consulting clients on ways to better network security, building data
centers and ISP's. Wrote programs to help CSR's take down data more efficiently and to allow
them to process orders. Wrote e-commerce web based forms for customers and clients to easily
transfer moneys securely via the internet. Written and released a Linux based distribution used
for secure firewalls, mail, file stores, and VPNS for our clients.

Accomplishments:
Direct Sentry - Based on the Slackware distribution of Linux I created my own Distribution of linux
for easy install. Included in current version are Firewall, IDS, Windows file sharing, Web Filtering,
Mail, etc...

VPN - Created VPN(Virtual Private Network) scripts to connect multiple offices. Wrote scripts
using bash scripting. Using these scripts to connect Doctor Offices, Insurance Companies,
Schools, and Telephone Companies offices to each other so they would be able to share network
resources.
ISP Build Outs - Built from ground-up, using both Windows and Linux based servers, networks for
ISP's throughout the United States. Setup Cisco Routers, Portmaster Modem pools , Webservers
Mail servers, Monitoring and Billing servers.
Virtual Linux Server Project - Using the Virtual Linux Server project created a Linux cluster of 5
computers sharing all resources. This was used as redundant mail server for a large scale ISP.
Machines were loaded with Slackware Linux and were previously workstations for company
employees. The machines would check each others system stats and report to the monitoring
station with then paged a technician if problem was not resolve by system restart. The offending
machine when then be taken automatically out of the cluster queue until repairs were made.
Credit Card Processing - Using PHP scripting, JavaScript and HTML designed multiple web
based forms for the Customer Service Reps to use when taking calls from people calling to order
Products from different call center clients. With these forms and scripts the amount charged was
put into the accounts of the call center clients.
Wireless Network - Built and designed a wireless network located in Eastern Idaho, established
firewalls, monitoring points and access lists to allow smooth run of traffic. A total of 8 cities and 13
Access Points. Utilized 2.4 gig radios and 5.8 gig radios.
