FOR A CERTIFICATE OF PUBLIC CONVENIENCE AND NECESSITY TO PROVIDE LOCAL EXCHANGE SERVICES IN THE STATE OF IDAHO CASE NO. MAT-07- NOTICE OF APPLICATION NOTICE OF MODIFIED PROCEDURE ORDER NO. 30288 YOU ARE HEREBY NOTIFIED that on January 16, 2007, Matrix, Inc. filed an Application for a Certificate of Public Convenience and Necessity to provide facilities-based and resold local exchange telecommunications services within the State of Idaho. The Application states Matrix will provide these services through interconnection agreements and commercial wholesale agreements with Verizon Northwest Inc. and Qwest Corporation. Matrix anticipates it will begin providing service in the spring of 2007 and intends to deploy and maintain company- owned facilities only after customer saturation can support such deployment. YOU ARE FURTHER NOTIFIED that Matrix has a Certificate of Incorporation from the State of Texas dated June 30 , 1990 and a Certificate of Authority from the State of Idaho dated June 12, 1992. The Company s principal place of business is in Dallas, Texas. Matrix has provided fmancial information and the appropriate contact information for its registered agent for service of process within Idaho. The Company also filed a revised illustrative tariff on February 21 2007 and affirms that it has reviewed all the Commission rules and agrees to comply with them. YOU ARE FURTHER NOTIFIED that the Commission has determined that the public interest may not require a formal hearing in this matter and will proceed under Modified Procedure pursuant to Rules 201 through 204 of the Idaho Public Utilities Commission s Rules of Procedure, IDAPA 31.01.01.201-204. The Commission notes that Modified Procedure and written comments have proven to be an effective means for obtaining public input and participation. NOTICE OF APPLICATION NOTICE OF MODIFIED PROCEDURE ORDER NO. 30288 YOU ARE FURTHER NOTIFIED that any person desiring to state a position on this Application may file a written comment in support or opposition with the Commission within twenty-one (21) days from the service date of this Notice. The comment must contain a statement of reasons supporting the comment. Persons desiring a hearing must specifically request a hearing in their written comments. YOU ARE FURTHER NOTIFIED that all proceedings in this matter will be conducted pursuant to the Commission s Rules of Procedure, IDAPA 31.01.01.000 et seq. ORDER IT IS HEREBY ORDERED that the Application of Matrix, Inc. for a Certificate of Public Convenience and Necessity to provide local exchange telecommunications services within the State of Idaho be processed by Modified Procedure, IDAPA 31.01.01.201-204. Persons interested in submitting written comments in this matter must do so no later than 21 days from the service date of this Order.
