Enclosed for fiing please find the original copy of the Notice of name change submitted on
behalf of Matrix Telecom, Inc. d/b/a Matrix Business Technologies also d/b/a Trinsic
Communications. Attached are the copies of the Secretary of State for Matrix Telecom, Inc.,
Matrix Business Technologies and Trinsic Communications. Per Kim Hunter at the Secreta
of State, they do not add d/b/a's to the original Secretary of State. The company also
respectfully requests an update to the CPCN reflecting the new name.
Business Customers wil be biled under the Matrix Business Technologies assumed name
and all Residential customers are to be biled under the Trinsic Communications assumed
naie. All customers have been noticed and a copy is attached. The company's updated price
lists/tariffs are being submitted separately to indicate the addition of the Trinsic
Communications name.
