On March 24,2016,Matrix Telecom,Inc.and TNCI Operating Company,LLC
(“TNCI OpCo”)filed an Application asking the Commission to:(1)amend and reissue the
Certificate of Public Convenience and Necessity (CPCN)for Matrix Telecom,Inc.to reflect that
Matrix Telecom,Inc.has changed its name to Matrix Telecom,LLC;(2)transfer the customers
of TNCI OpCo to Matrix Telecom LLC;and (3)cancel TNCI OpCo’s CPCN.
IT IS HEREBY CERTIFIED that this Second Amended Certificate of Public
Convenience and Necessity,Certificate No.468,is issued to Matrix Telecom,LLC.This
Certificate authorizes Matrix to provide local exchange and interexchange services within the
State of Idaho.The Commission approved Matrix’s Certificate by Order No.33517 issued May
3,2016,and authorized Matrix to provide telecommunications services in the State of Idaho.
This Certificate is predicated upon and is issued pursuant to the findings of the Commission in
Order No.33517.
